Requirements

  • A valid New York State driver's license, a personal car for employment duties, and an active automobile insurance policy are required.
  • Basic computer skills are required.
  • Good writing and verbal skills are required.
Responsibilities
  • Identify high-risk prenatal and postpartum women.
  • Assist individuals in accessing primary and preventive health services, including prenatal care, PCAP, WIC, and other essential and supportive services as needed.
Desired Qualifications
  • Spanish or Creole speaking ability is a plus.
