Requirements
- A High School diploma or GED is required.
- At least one year of full-time experience working with individuals who receive behavioral health treatment services is required.
- A valid driver's license and two years of driving experience are required.
- Candidates must be conscientious and dependable, have good computer, communication, organizational, and time-management skills, work well with other staff, and be able to work independently.
- Candidates must maintain all required trainings and supervision and meet the applicable 908 KAR 2250 funding requirements for billable H2015 and Medicaid services.
Responsibilities
- Teach clients to utilize and access community resources, adhere to medications, develop interpersonal and behavioral support skills, cope with crises, regulate emotions, access transportation, budget and manage finances, and shop independently in the community.
- Complete documentation in the electronic medical record within 24 hours of service delivery.
- Conduct activities that enable individuals to live with maximum independence in the community.
- Enhance community living through skills training, cueing, or supervision identified in recipients' treatment plans.
- Participate in therapeutic rehabilitation and 1915i Rise services as needed.
