Requirements
A High School diploma or GED is required.
At least one year of full-time experience working with individuals who receive behavioral health treatment services is required.
A valid driver's license and two years of driving experience are required.
Candidates must be conscientious and dependable, have good computer, communication, organizational, and time-management skills, work well with other staff, and be able to work independently.
Candidates must maintain all required trainings and supervision and meet the applicable 908 KAR 2250 funding requirements for billable H2015 and Medicaid services.
Responsibilities
Teach clients to utilize and access community resources, adhere to medications, develop interpersonal and behavioral support skills, cope with crises, regulate emotions, access transportation, budget and manage finances, and shop independently in the community.
Complete documentation in the electronic medical record within 24 hours of service delivery.
Conduct activities that enable individuals to live with maximum independence in the community.
Enhance community living through skills training, cueing, or supervision identified in recipients' treatment plans.
Participate in therapeutic rehabilitation and 1915i Rise services as needed.

Credentials: High School diploma or GED with one year of full-time experience working with individuals who receive services for treatment of a behavioral health disorder. Must have valid driver's license with 2 years of driving experience.
Special skills: Conscientious, Dependable, Good computer skills, Works well with other staff, Good communication skills. Ability to work independently. Good organizational and time management skills.
Primary functions:
Teaching clients the following skills: how to utilize/access community resources, medications adherence, interpersonal skills, behavioral support skills, crisis coping skills, emotional regulation skills, how to access transportation, how to budget/manage finances, and how to shop independently in the community.
Complete documentation in EMR within 24 hours of service delivery.
Conduct activities necessary to allow individuals to live with maximum independence in the community.
Enhance community living through utilizing skills training, cueing or supervision as identified in recipients treatment plan.

Shift/Days & Work Hours: Monday - Friday 8:00AM - 4:00PM
