Requirements
  • A bachelor's degree in a human services field.
  • A strong background in admission or intake in a health care organization, or experience with the Office for People with Developmental Disabilities system.
  • Ability to communicate effectively with individuals and team members while appreciating their social, cultural, religious, and ethnic backgrounds and their age, gender, and abilities.
  • Ability to uphold personal and professional ethics and values while respecting the culture, beliefs, and abilities of individuals served.
  • Ability to develop long-standing relationships with clients, their families, and national stakeholders.
  • Ability to work in a team-oriented environment and contribute to a multidisciplinary team.
  • Ability to demonstrate initiative and maintain a stable level of productivity.
Responsibilities
  • Participate in screenings of selected candidates.
  • Coordinate screenings and intakes with screening teams.
  • Ensure candidates are approved by the New York State Office for People with Developmental Disabilities, eligible for Medicaid, and enrolled in Home and Community-Based Services.
  • Collect data and conduct certain types of assessments.
  • Review referral packets for placement in residential, day, and therapeutic service programs.
  • Collect historical information before screening, admission, or intake.
  • Generate and update reports for residential vacancies.
  • Conduct questionnaires and complete documents necessary to support people receiving residential, day, crisis, and therapeutic services.
  • Disseminate and follow up on consents required for admission or service.
  • Attend AROC meetings to facilitate program placement for individuals in need of services.
  • Contact family members or circles of support to arrange Meet & Greets, pre-planning meetings, and admission entrance meetings.
  • Correspond with the Office for People with Developmental Disabilities regarding admission and intake needs.
  • Assist with weekly calls to families and circles of support to relay updates.
  • Assist with referrals and intake for CSIDD and IPSIDD clinical services.
  • Attend aging-out meetings and review reports.
  • Correspond with CCO Care Managers regarding community services and Life Plans.
  • Complete the enrollment process in the agency's electronic health record.
  • Enter diagnoses, admissions, and internal transfers into the agency's electronic health record.
  • Enter information into Choices, including adding and removing entries.
  • Ensure intake data is collected and documented accurately on PrecisionCare, Power BI, and other agency electronic health platforms.
  • Maintain a comprehensive and accurate system of individual records and statistical data.
  • Collaborate with residential, day services, crisis services, and IPSIDD clinical teams.
  • Participate in Office for People with Developmental Disabilities conference calls to provide updates on new development projects.
  • Collaborate with the Coordinator of Intake, Program Directors, and Treatment Coordinators.
  • Consult with team members about programs and services for people supported.
  • Attend and coordinate meetings regarding people supported.
  • Assist with scheduling needs for residential, day, CSIDD, and IPSIDD services.
  • Perform special projects, including dead-file processing, ordering supplies, and coordinating special activities.
  • Work directly with families, people supported, and circles of support to explain the intake and admission process and service offerings.
  • Provide support to families by attending meetings or corresponding with them.
Desired Qualifications
  • Two years of experience working with people with intellectual and developmental disabilities and behavioral health needs.
  • A master's degree in an applied area of psychology, social work, or special education.
  • Experience working in admissions or a health office.
  • Proficiency with Microsoft Word, Microsoft Excel, and PowerPoint.
  • Proficiency in using electronic health records.
  • Strong writing and organizational skills.
  • Excellent time management skills to complete multiple tasks simultaneously.
