Requirements
  • A current license and registration to practice as a Licensed Practical Nurse in New York State are required.
  • Ability to provide nursing care within the Licensed Practical Nurse scope of practice and under appropriate Registered Nurse direction.
  • Ability to direct, support, and work collaboratively with Certified Nursing Assistants and other members of the care team.
  • Strong verbal and written communication skills.
  • Sound problem-solving and decision-making skills.
  • Ability to document accurately and use required clinical computer systems.
  • A caring, patient, and respectful approach to residents.
  • Physical stamina for an active, hands-on nursing position.
  • Ability to read, write, speak, and understand English at the level necessary for safe care and effective workplace communication.
Responsibilities
  • Provide direct, high-quality nursing care.
  • Plan, implement, and evaluate resident care.
  • Maintain regulatory compliance and quality standards.
  • Collaborate effectively with the care team.
  • Administer medications and ordered treatments safely and accurately.
  • Manage the daily clinical care of assigned residents and short-stay rehabilitation patients.
  • Monitor residents closely and promptly communicate symptoms, changes in condition, clinical concerns, and progress to the appropriate Registered Nurse or provider.
  • Follow each resident’s plan of care and contribute clinical information needed to maintain accurate, individualized care.
  • Direct and monitor the personal-care duties and nursing-care procedures completed by Certified Nursing Assistants on the assigned team.
  • Provide coaching, guidance, and hands-on assistance to Certified Nursing Assistants when resident or unit needs require it.
  • Maintain accurate nursing notes, medication-administration records, treatment records, and other required documentation.
  • Safeguard, store, administer, and account for medications and controlled substances according to policy and regulatory requirements.
  • Communicate professionally with residents, families, providers, rehabilitation professionals, and other members of the interdisciplinary team.
  • Support infection prevention, resident safety, regulatory compliance, and a respectful care environment.

  • Previous nursing experience in a skilled nursing, rehabilitation, long-term-care, or geriatric setting.
  • Experience caring for short-stay rehabilitation patients.
  • Experience with dementia or memory-care populations.
  • Experience supporting complex treatments and clinically diverse resident needs.

Elderwood partners with Trocaire College to advance nursing careers of current employees. Scholarship program provides both financial support and mentorship. Trocaire College in partnership with Elderwood, announced the launch of the Elderwood-Trocaire iCARE Career Pathway Scholarship. This initiative represents a $150,000 annual investment by Elderwood to support up to ten employees at a time as they pursue a practical nursing (LPN), registered nursing (RN), or bachelor's degree in nursing (BSN) at Trocaire. "This new scholarship program is another opportunity for Trocaire and Elderwood to work together to help strengthen the local healthcare workforce," said Dr. Bassam Deeb, president, Trocaire College. "The demand for qualified nurses locally continues, and with Elderwood's commitment to investing in its employees, this program provides financial support, mentorship, and a clear path forward for those looking to advance in their healthcare careers." Beyond financial support, the partnership between Trocaire and Elderwood provides mentorship and guidance from experienced clinical and operational leaders, as well as helping the participants successfully navigate their education and transition into advanced nursing roles. Full-time and part-time academic program opportunities are available. Employees must continue working at least 15 hours per week while enrolled at Trocaire and commit to one-year of employment with Elderwood upon program completion. "We see this partnership as more than a scholarship, it's an investment in people who are already committed to caring for others," said Dr. Jeffrey Rubin, co-CEO of Post Acute Partners, owner of Elderwood. The team and Legends Global are holding a job fair for culinary and hospitality positions Monday, June 15th from 4pm-7pm at the team's fieldhouse across the street from the new stadium. 500 people are needed to be cooks, dishwashers, supervisors, attendants, bartenders and much more.
