Requirements
  • At least 2 years of experience as a social work technician (TTS).
  • A college diploma in social work recognized by Quebec's Ministry of Education, or an equivalent recognized by MIFI.
Responsibilities
  • Assess or review the client's needs.
  • Develop or revise the client's intervention plan (PI/PII) or individualized service plan (PSI), and submit accommodation requests.
  • Help direct the client to an appropriate resource and residential organization; coordinate client-resource matching in collaboration with DEQEPE.
  • Integrate the client into the resource and support their integration into the new living environment.
  • Follow up with the legal representative or family.
  • Monitor client budgets.
  • Enter follow-up notes in I-CLSC.
  • Collaborate with internal and external partners and make referrals as needed.
  • Collaborate in assessing and reassessing the need for representation assistance.
  • Determine the particular support or assistance services to provide to the client using the classification instrument, and complete the classification within 30 to 60 days following placement.
  • Prepare the OCCI when an accommodation request is made.
  • Implement the client's intervention plan (PI) or individualized service plan (PSI).
  • Coordinate the client's departure from the resource.
  • Collaborate in applying the quality-control process.
  • Complete or update the client's personal summary and provide it within 72 hours following placement.
