Requirements
  • Possess a Nursing Degree from an accredited college or university, or graduate from an approved Licensed Practical Nurse/ Licensed Vocational Nurse program.
  • Possess a current, unencumbered, active license to practice as a Licensed Practical Nurse/ Licensed Vocational Nurse in the state.
  • Read, write, speak, and understand English; read technical procedures and policy and procedure manuals.

  • Assist with resident evacuation during emergency situations when necessary.
Responsibilities
  • Provide direct nursing care to residents and supervise the day-to-day nursing activities performed by nursing assistants.
  • Direct the day-to-day functions of nursing assistants and ensure that nursing personnel comply with facility policies, procedures, job descriptions, and applicable regulations.
  • Plan shift services, programs, and activities with assigned nursing staff and support personnel.
  • Review and recommend revisions to department policies, procedure manuals, and job descriptions.
  • Prepare written and oral reports and recommendations concerning shift activities.
  • Coordinate nursing services with other resident services to maintain each resident’s total care regimen.
  • Participate in the nursing service quality assurance program and facility surveys conducted by authorized government agencies.
  • Review and update resident discharge plans and assist with planning nursing services for discharge.
  • Interpret department policies and procedures for personnel, residents, visitors, and government agencies.
  • Admit, transfer, and discharge residents and complete related records, forms, and reports.
  • Maintain census reports, charge slips, medical forms, evaluations, studies, charts, and other administrative records.
  • Protect resident health information, user credentials, and facility information systems and report suspected unauthorized access.
  • Complete admission, transfer, discharge, accident, incident, and routine charting documentation.
  • Receive telephone orders from physicians and transcribe physician orders to resident charts, medication cards, treatment plans, and care plans.
  • Chart nursing notes descriptively, record resident responses to care, and report discrepancies in orders, diet changes, and documentation.
  • Prepare and administer physician-ordered medications and treatments after verifying resident identity.
  • Maintain medication, narcotic, supply, and equipment inventories; order supplies as needed and report discrepancies.
  • Review medication cards, monitor stop orders, notify physicians when required, and dispose of drugs and narcotics according to procedures.
  • Participate in employee performance evaluations, determine shift staffing needs, and make staffing recommendations to the Nurse Supervisor.
  • Develop work assignments, provide leadership to nursing personnel, conduct unit rounds, and report problem areas.
  • Ensure that nursing assistants and nurse aide trainees meet training and supervision requirements.
  • Conduct regular meetings with nursing personnel to identify and correct problem areas and improve services.
  • Receive and give nursing reports at the beginning and end of shift duty.
  • Report occupational exposures, fraud, resident incidents, staffing issues, and other required matters.
  • Inform nursing personnel of new admissions, prepare rooms, greet and orient residents, and coordinate admission activities.
  • Make physician rounds, arrange diagnostic and therapeutic services, consult physicians, and review resident charts for treatments, medications, and diets.
  • Implement nursing objectives and standards and verify that prescribed treatments are properly administered.
  • Provide or oversee direct nursing care, including catheterization, tube feedings, suctioning, dressing changes, colostomy and drainage-bag care, specimen collection, vital signs, restorative programs, and range-of-motion exercises.
  • Use restraints, monitor seriously ill residents, and respond to changes in resident condition according to policy.
  • Coordinate social and activity programs with nursing schedules and notify physicians and next of kin of accidents or changes in condition.
  • Arrange transportation and escort discharged residents; notify families of resident deaths and follow post-mortem procedures.
  • Develop, schedule, and conduct in-service training and orientation for nursing personnel.
  • Assist with training related to blood and body fluid exposure and clinical supervision for nurse aide trainees.
  • Attend required facility, continuing education, infection control, safety, and advance directive training programs.
  • Monitor personnel compliance with safety, sanitation, hand hygiene, infection control, fire safety, disaster preparedness, and personal protective equipment procedures.
  • Maintain clean and sanitary nursing work areas and resident care areas.
  • Ensure adequate supplies, medications, equipment, personal protective equipment, and material safety data sheets are available.
  • Ensure trained and authorized personnel operate nursing equipment safely and efficiently.
  • Review resident care plans daily, identify needed changes, and ensure nursing notes document care-plan implementation.
  • Ensure certified nursing assistants understand and follow resident care plans.
  • Assist with Minimum Data Set assessments, Resident Assessment Protocols, and care-plan scheduling and revisions.
  • Provide data to the Quality Assurance and Assessment Committee as requested.

PACS Group has appointed Carey P. Hendrickson as chief financial officer, effective 27 April 2026. Hendrickson brings nearly four decades of financial leadership across healthcare, senior living and media sectors. PACS Group has announced that co-founder and chief financial officer Mark Hancock will retire on 30 June 2026. He will continue serving on the company's board of directors as vice chairman.
