Requirements
  • A high school diploma or GED is required.
  • At least one year of experience in human services is required.
  • Experience in medical care and interaction with medical professionals is required.
  • Excellent oral and written communication skills and the capacity and willingness to learn computer systems are required.
  • A valid driver's license is required, including the ability to drive large vehicles such as wheelchair-accessible or 15-seat vans.
  • Successful completion and maintenance of AMAP certification is required.
  • SCIP-R certification, cardiopulmonary resuscitation certification, and first aid certification are required.
Responsibilities
  • Provide and ensure a safe, clean, normalized, home-like environment and inform the Program Manager of physical plant needs.
  • Promptly inform the Program Manager or Assistant Program Manager of serious incidents or violations of resident or staff rights according to established policies.
  • Develop residents' activities of daily living and independent living skills.
  • Maintain knowledge of each resident's medical, psychological, cognitive, and social needs and ensure programming is appropriate to their needs and treatment goals.
  • Maintain and submit timely and accurate records and reports concerning residents' development status, progress, daily goal data, monthly progress notes, summaries, and other required information.
  • Maintain and check medication administration records daily as needed.
  • Maintain and order individual medications according to doctors' orders as needed.
  • Schedule individuals' medication appointments.
  • Review individuals' medical records to ensure all appointments are followed.
  • Communicate daily with nurses and management regarding individuals' appointments and follow-ups.
  • Participate in interdisciplinary treatment meetings and regularly scheduled resident reviews as required.
  • Provide habilitation services to each client as prescribed in the client's service plans, including the CFA, Individual Service Plan, and Habilitation Plan.
  • Assist with transporting individuals to and from programs, community inclusion activities, medical appointments, home visits, and other activities as needed.
  • Assist clients with skill development in activities of daily living, domestic living, recreation, and socialization.
  • Immediately report observed or known deficiencies, inability to provide habilitation services, abuse, facility problems, and physical plant or safety issues to management.
  • Ensure assigned clients' nutritional needs are met according to prescribed diets and assist with feeding as necessary.
  • Organize, assist with, supervise, and when necessary prepare residents' meals.
  • Assume responsibility for agency or resident funds personally assigned for resident or program purchases.
  • Escort individuals to scheduled medical appointments.
  • Complete the Staff Information Entry Form when escorting individuals to medical appointments.
  • Communicate pertinent resident and program issues verbally and in writing to the Program Nurse and supervisors, including medical consult reports, incident reports, and the residence logbook, and read the residence logbook daily.
  • Maintain knowledge of each resident's medical appointments and follow-up needs.
  • Assist the Housekeeper/Cook with household cleaning and facility maintenance.
  • Meet attendance standards according to assigned shifts at the discretion of the Program Manager or Assistant Program Manager.
  • Successfully participate in and pass medication certification courses and demonstrate continued proficiency in medication administration according to established policies.
  • Perform housekeeping duties assigned by the Program Manager.
  • Perform related duties and other duties as assigned by the Program Manager.
  • Maintain awareness of and sensitivity to the cultural and socioeconomic characteristics of the persons and population served.
