About the job

Requirements
  • A High School Diploma or GED with at least 3 years of experience is required.
  • Experience working with substance abuse, homelessness, families, and groups is required.
  • Excellent written and oral communication skills are required.
  • Basic computer skills are required.
Responsibilities
  • Create a resource bank that will serve all HomeBase clients.
  • Meet with clients who may need more intensive services to access resources and adjust to the community.
  • Attend appointments other than court with clients who need additional instructions.
  • Complete home visits for Aftercare clients.
  • Accompany the Outreach Specialist to presentations to acquire additional resources.
  • Assist families with identifying and establishing relationships with supports.
  • Act as a liaison between families, case managers, and other providers.
  • Provide needed services through referrals and follow up on progress.
  • Assist the Housing Specialist with establishing and maintaining relationships with landlords and brokers willing to work with HomeBase clients.
  • Maintain a tracking system for renewals of City rental subsidies.
