Aftercare Specialist
About the job
- A High School Diploma or GED with at least 3 years of experience is required.
- Experience working with substance abuse, homelessness, families, and groups is required.
- Excellent written and oral communication skills are required.
- Basic computer skills are required.
- Create a resource bank that will serve all HomeBase clients.
- Meet with clients who may need more intensive services to access resources and adjust to the community.
- Attend appointments other than court with clients who need additional instructions.
- Complete home visits for Aftercare clients.
- Accompany the Outreach Specialist to presentations to acquire additional resources.
- Assist families with identifying and establishing relationships with supports.
- Act as a liaison between families, case managers, and other providers.
- Provide needed services through referrals and follow up on progress.
- Assist the Housing Specialist with establishing and maintaining relationships with landlords and brokers willing to work with HomeBase clients.
- Maintain a tracking system for renewals of City rental subsidies.
