Requirements
- A high school diploma or equivalent is required.
- Experience working with individuals with developmental and intellectual disabilities is required.
- Knowledge of person-centered planning and community integration principles is required.
- A valid driver's license, reliable transportation, and valid insurance are required.
- The ability to maintain strict confidentiality and adhere to ethical guidelines is required.
Responsibilities
- Transport participants to and from daily programs.
- Assess individual needs and develop personalized plans to enhance community integration.
- Facilitate skill-building activities to promote independence and socialization.
- Coordinate and accompany individuals on community outings and activities.
- Provide support and assistance with daily living tasks as needed.
- Collaborate with families, caregivers, and service providers to ensure effective implementation of plans.
- Monitor progress and document outcomes according to agency standards.
