Gentiva provides in-home hospice, palliative, and home health care across the United States through a national network. Care is delivered by clinicians and therapists in patients’ homes, including an Advanced Illness Management model for those not yet hospice-eligible. The company differentiates itself with an integrated at-home care platform and strategic acquisitions, such as ProMedica’s home health, palliative, and hospice operations. Its goal is to help patients stay at home, reduce avoidable hospitalizations, and support families and caregivers throughout the care continuum.
