Requirements
- A Master's Degree in Nursing from an accredited college or university is required, along with certification as a Nurse Practitioner in the area of clinical focus.
- A minimum of three years of nursing experience is required, including experience in oncology, hospice, geriatrics, or palliative medicine.
- Certification in palliative care, Certified Hospice and Palliative Nurse, or Advanced Certified Hospice and Palliative Nurse is required within 24 months of hire or transfer.
- Basic Life Support certification is required every two years.
Responsibilities
- Provide palliative medicine consultation for seriously ill patients and their families as a member of the Palliative Interdisciplinary team.
- Perform comprehensive medical and psychosocial evaluations of patients and families.
- Provide skilled symptom management.
- Participate in discharge planning and continuity-of-care efforts according to Northside Hospital standards of care.
- Conduct educational efforts in palliative medicine.
- Promote excellence in care and customer service to all patients served.
- Serve as a role model of excellence in nursing knowledge and clinical practice.
Desired Qualifications
- Advanced Cardiovascular Life Support certification is preferred and must be renewed every two years.
