Requirements
  • A Master of Social Work, Master in Professional Counseling, or Master of Family Therapy degree from a nationally accredited institution, fulfilling the requirements for licensure in Oregon and/or Washington.
  • A minimum of two years of clinical experience or experience in a healthcare-related field.
  • Ability to diagnose common mental health problems and conduct solution-focused interventions using applicable modalities including Acceptance and Commitment Therapy, Cognitive Behavioral Therapy, Cognitive Behavioral Therapy for Insomnia, and psychodynamic psychotherapy.
  • Ability to recognize and advocate for patients regarding behavioral health strategies and interventions.
  • Ability to promote health behavior change through evaluation, brief intervention, and education.
  • Ability to work with a multidisciplinary team, coordinate with providers and community partners, and refer patients for appropriate mental health therapy.
  • Competency in assessing emotional, cognitive, and behavioral disorders in child, adolescent, adult, and/or geriatric patients, depending on the population served.
  • Competency in assessing cognitive and psychological problems associated with or exacerbating medical conditions.
  • Experience working with an interdisciplinary team.
  • Skill in case conceptualization and consultation.
  • Flexibility and willingness to work at the fast pace of primary care.
  • A current applicable state license as a Clinical Social Worker, Licensed Independent Clinical Social Worker, Licensed Professional Counselor, Licensed Mental Health Counselor, or Licensed Marriage and Family Therapist.
Responsibilities
  • Deliver advanced-level, evidence-based behavioral health interventions targeted to improve patients' overall health.
  • Meet individually with patients for assessment and provide recommendations to the primary care team to enhance each patient's care plan.
  • Provide subsequent sessions when indicated, including skills training and coaching to improve patients' health and functioning.
  • Help patients who require services not provided in the clinic transition smoothly to care with community partners.
