Requirements
  • Graduate of an accredited School of Practical Nursing as a Licensed Practical Nurse or Licensed Vocational Nurse.
  • Hold current appropriate state licensure.
  • Have a minimum of 2 years of hemodialysis experience.
  • Successfully complete a training course in the theory and practice of hemodialysis.
  • Successfully complete CPR certification.
  • Be able to work independently to manage emergencies.
  • Complete the Nurses Technical Training Program upon hire or soon thereafter.
  • Successfully complete the Staff Assisted Home Hemodialysis Home Caregiver Orientation and Training.
  • Have good verbal and written communication skills.
  • Employees must complete the Ishihara's Color Blindness test as a condition of employment; failing the test does not preclude employment because reasonable accommodations may be considered.
Responsibilities
  • Provide direct patient care and administer intravenous medication to assigned home patients under the direct supervision of a Registered Nurse, in accordance with physician orders and state nurse practice laws.
  • Assist the Home Therapy Supervisor in identifying, evaluating, and training candidates for the home dialysis program.
  • Accurately document patient training and treatment information in the medical record and Medical Information System.
  • Monitor, assess, and intervene for home patients, including conducting home visits, additional training sessions or reviews, lab studies, and annual X-ray and EKG studies.
  • Maintain compliance with patient supply and inventory procedures, document appropriately, and maintain durable medical equipment, including preventive maintenance and water analysis.
  • Perform monthly chart audits and report the results to the supervisor.
  • Report outcomes to the Home Therapy Manager or Coordinator and attending physicians as appropriate.
  • Revise the Patient Plan of Care with signed physician orders under the direction of the Home Therapy Manager or Coordinator.
  • Schedule backup treatments when necessary in collaboration with the Home Therapy Manager and physician.
  • Educate patients regarding treatment options as assigned.
  • Maintain current knowledge of dialysis equipment used in the program and field, including hemodialysis machines and water treatment systems.
  • Troubleshoot equipment problems with technical personnel by telephone.
  • Participate in scheduled staff meetings and patient care conferences as appropriate.
  • Review the Patient Plan of Care as required.
  • Participate in scheduled in-service classes covering current dialysis principles and techniques.
  • Provide educational in-services about home therapies to in-center staff, hospitals, or nursing programs as assigned.
  • Act as a resource for other staff members.
  • Travel between the facility, patients' homes, and various community locations, with possible travel to regional, Business Unit, and Corporate meetings.
  • Perform other duties as assigned.
