Requirements
  • Perform personal care according to each patient's assignment and report tasks that cannot be performed to the Registered Nurse Case Manager.
  • Meet patient safety needs and use equipment safely and properly, including foot stools, side rails, and oxygen equipment.
  • Perform vital signs and activities of daily living as needed, including bathing, oral hygiene, shampooing, changing bed linen, dressing, undressing, and feeding assistance.
  • Provide skin care and observation to prevent tissue breakdown over bony prominences.
  • Report patient conditions and significant changes to the Registered Nurse Case Manager and remain aware of caregiver and household interpersonal issues.
  • Assist with ambulation and exercise as instructed by the hospice nurse or therapist.
  • Assist with bedpans, urinals, and light laundry needs as assigned.
  • Perform range-of-motion exercises and other simple procedures ordered as extensional therapy services after demonstrating competency.
  • Provide respite for patients, families, or caregivers when on site, as appropriate.
  • Keep the patient's immediate living space tidy as assigned.
  • Follow organizational documentation, care procedures, and personal and professional conduct standards.
  • Participate in quality assessment and performance improvement teams and activities.
  • Respect the patient's and family or caregiver's environment and personal needs.
  • Participate in on-call duties as assigned.
  • Meet state training requirements.
  • Hold an active Certified Nursing Assistant license in the state where applying and working.
  • Have one year of Certified Nursing Assistant experience.
  • Be at least 18 years of age.
  • Read and follow written instructions and document care provided.
  • Work independently with little direct supervision and maintain a calm manner in patients' homes.
  • Demonstrate empathy for ill, injured, frail, and impaired individuals.
  • Maintain current cardiopulmonary resuscitation certification.
  • Maintain tact, patience, and good personal hygiene.
  • Hold a valid driver's license and have an insured, roadworthy automobile meeting organizational requirements.
  • Do not perform duties constituting the practice of nursing under the Nurse Practice Act.
  • Work in a community home environment with exposure to infectious diseases and travel in and out of an automobile.
  • Demonstrate oral and written communication skills.
  • Demonstrate organizational skills.
Responsibilities

  • Provide services under Registered Nurse supervision to meet patients' personal needs and promote comfort.
  • Perform assigned home health patient-care services in a manner that promotes quality, continuity, and safety of care.
Desired Qualifications
  • One year of home health or hospice experience is preferred.
