Requirements
- A current pharmacist license through the West Virginia State Board of Pharmacy is required for services provided in West Virginia.
- Obtain Basic Life Support certification within 60 days of the hire date.
- Complete the West Virginia Credentialing Form application to qualify to bill for services.
- Walk and stand for long periods of time.
- Lift 30 pounds and push 50 pounds.
- Use dexterity to finger, handle, and reach.
- Demonstrate time management skills.

Responsibilities
- Provide pharmaceutical care through direct patient care, operational, education, and administrative activities.
- Monitor patients’ drug therapy by participating in medical rounds or evaluating medication regimens, medical problems, laboratory data, and special procedures.
- Provide leadership and patient care in assigned areas, guide providers and patients on established care practices, and assure the safe and effective use of drugs through clinical involvement.
- Assist nursing and other disciplines or departments with admission and discharge planning and patient education related to medication use.
- Provide drug information and educational programs to medical and nursing staff, other health care practitioners, and pharmacy staff.
- Maintain professional competence by attending and presenting at conferences, completing continuing education, reading current professional literature, and publishing clinical or practice-related experience or research.
- Participate in administrative, quality assurance, performance improvement, clinical research, and medication use evaluation activities.
- Review medication orders for accuracy and appropriateness.
- Evaluate compliance with medication safety procedures, suggest improvements based on best practices such as the Institute for Safe Medication Practices, document medication errors and near misses, and promote medication safety.
- Dispense medications from provider orders and ensure compliance with drug distribution policies and procedures.
- Control drug use in assigned service areas and identify and help resolve related problems.
- Make decisions related to pharmaceutical services in the assigned service area.
- Detect, report, and help minimize or manage adverse drug reactions and medication errors.
- Participate in management of acute and chronic disease states, medical emergencies, and adverse drug reactions.
- Communicate relevant findings and recommendations and document them in the clinical information system.
- Maintain required records in accordance with legal requirements and hospital policies and procedures.
- Supervise technicians in drug preparation and distribution duties.
- Provide objective information on co-worker performance to facilitate performance management.
- Participate in orientation and education of pharmacy students, pharmacy residents, and new department employees.
- Participate in educational and scholarly activities related to drug therapy.
- Participate in planning and implementing pharmacy changes and improving productivity through staff meetings and projects.
- Complete and submit written assignments, continuing education documentation, and other assigned items on time.
- Complete annual clinical competency evaluations.
- Document patient encounters and billing-related information in the electronic medical record.
- Obtain and maintain organizational and payer credentialing.
- Collaborate with pulmonology providers to optimize medication management, support patient care, and enhance clinic workflows.
- Establish pharmacy presence in the clinic through a collaborative practice agreement, provide direct patient care and medication education, and manage chronic diseases including chronic obstructive pulmonary disease, asthma, pulmonary fibrosis, and interstitial lung disease.
- Precept and mentor student and resident learners.
Desired Qualifications
- A Doctor of Pharmacy (Pharm.D.) degree.
- Completion of a postgraduate year-two residency in an applicable clinical field.
- Completion of a postgraduate year-one residency and board certification in an applicable clinical field.
- Completion of a postgraduate year-one residency and three years of relevant pharmacist experience.
- Three years of relevant pharmacist experience and board certification in an applicable clinical field.
