Requirements
  • A Bachelor of Science in Nursing degree is required.
  • Current New York State Registered Nurse licensure is required.
  • Three to five years of healthcare experience in an acute care setting is required.
  • Superior communication, demonstrated problem-solving skills, strong business acumen, and interpersonal skills are required.
Responsibilities
  • Provide intensive care management for adult patients with complex medical needs.
  • Serve as a liaison between the interdisciplinary team of social workers, physicians, physical therapists, occupational therapists, cardiologists, and intensivists, supporting patients and their families.
  • Direct all aspects of inpatient care and develop individualized care plans to ensure optimal length of stay.
  • Help prevent hospital readmission and anticipate outpatient service needs by setting appointments and consultations prior to discharge.
Desired Qualifications
  • Two years of case management or care coordination experience in an inpatient, HMO, insurance, or other outpatient setting is preferred.
  • One year of leadership experience in a healthcare setting is preferred.
  • A Master's degree in a healthcare-related field is preferred.
  • Fluency in Spanish is preferred.
