- Graduate of an accredited Physical Therapy program with a Doctor of Physical Therapy, Master of Science, or Bachelor of Science degree.
- Hold an active Ohio Physical Therapist license or be eligible for one.
- Possess strong communication and teamwork skills.
- Be willing to work with geriatric and post-acute populations.
- Evaluate patients and develop individualized treatment plans.
- Provide evidence-based physical therapy interventions.
- Collaborate with physicians, nurses, and rehabilitation team members.
- Supervise Physical Therapy Assistants.
- Document patient progress accurately and timely.
- Participate in care planning and interdisciplinary meetings.
- Educate patients and caregivers on mobility and safety.
- Support discharge planning and functional outcomes.
- Experience in skilled nursing, rehabilitation, or post-acute care.
- Strong clinical decision-making and documentation skills.
- Ability to work independently and collaboratively.
