Requirements
  • A minimum of one year of experience as a registered nurse in acute care hospital, oncology, long-term care, hospice, or home health; school internships and clinical hours may be considered in individual cases.
  • A current license to practice nursing in the state of employment is required.
  • Current CPR certification is required.
  • Current automobile insurance and a valid driver's license are required.
  • The role requires knowledge of terminally ill patients and their families, hospice practices, hospice disciplines, community resources, general nursing practice, managed care principles, and regulatory guidelines including Medicare, Medicaid, and ACHC.
  • The role requires excellent patient assessment skills, documentation and program management knowledge, and effective oral and written communication.
  • The employee must be able to operate nursing assessment tools, computers, facsimile equipment, copiers, and cell phones or beepers, and must maintain transportation for work.
  • The employee must meet applicable state-specific requirements and work safely in a safety-sensitive position.
Responsibilities
  • Serve as the primary point of contact for patients and families throughout their hospice journey.
  • Deliver and document skilled nursing care according to individualized plans of care in patients' homes, assisted living facilities, or other locations where they reside.
  • Perform initial, ongoing, comprehensive, and reassessment screenings by telephone and in person, and provide education to patients, families, and caregivers.
  • Collaborate with physicians, licensed practical nurses, certified nursing assistants, social workers, chaplains, nurse practitioners, and other interdisciplinary team members to develop and update personalized care plans.
  • Provide emotional, physical, and spiritual support, education, comfort, and bereavement guidance to patients and their loved ones.
  • Teach caregivers safe care practices and provide practical and emotional support.
  • Deliver skilled nursing interventions and symptom management aligned with goals of care.
  • Develop and update individualized plans of care, including visit frequency and risk identification.
  • Communicate changes in patient condition and signs or symptoms of decline to appropriate providers, including nurse practitioners, primary care providers, and hospice physicians.
  • Provide education about disease processes, self-care techniques, end-of-life care, medications, nutrition, and dietary needs.
  • Support home health, palliative, and hospice referrals and transitions between levels of care.
  • Maintain timely and accurate clinical records, including assessment findings, physician orders, progress notes, and care plans.
  • Maintain effective working relationships with the interdisciplinary group and involve the team when the care plan requires broader participation.
  • Participate in on-call and patient support activities and conduct on-call services in a clinically competent and responsive manner.
  • Follow infection-control, compliance, regulatory, and policy standards.
  • Support quality-improvement and scheduling initiatives.
