Requirements

Responsibilities
  • Direct the day-to-day functions of nursing assistants in accordance with the rules, regulations, and guidelines governing the long-term care facility.
  • Ensure that assigned nursing personnel comply with facility policies, procedures, job descriptions, and the Nursing Service Procedures Manual.
  • Review policies, procedure manuals, and job descriptions and recommend revisions.
  • Plan shift services, programs, and activities with nursing and support personnel.
  • Prepare written and oral reports and recommendations concerning shift activities.
  • Coordinate nursing services with other resident services to maintain the resident’s total care regimen.
  • Participate in the development, maintenance, and implementation of the nursing service quality assurance program.
  • Participate in facility surveys conducted by authorized government agencies.
  • Review and update resident discharge plans and assist with the nursing services portion of those plans.
  • Interpret departmental policies and procedures for personnel, residents, visitors, and government agencies.
  • Admit, transfer, and discharge residents as required.
  • Complete accident and incident reports and maintain required census and administrative records.
  • Protect assigned user credentials, resident protected health information, and facility information systems.
  • Complete and file records and charts for resident admissions, transfers, and discharges.
  • Receive telephone orders from physicians and transcribe physician orders to resident charts, medication cards, treatment plans, and care plans.
  • Document nursing care, resident responses, accidents, incidents, diet orders, transfers, and other required information in the medical record.
  • Prepare and administer physician-ordered medications and treatments after verifying resident identity.
  • Maintain adequate medication, supply, and equipment inventory and report needs to the Nurse Supervisor.
  • Maintain accurate narcotic records and report medication or narcotic discrepancies.
  • Review medication cards for completeness and accuracy and notify physicians about automatic stop orders.
  • Dispose of drugs and narcotics according to established procedures.
  • Participate in employee performance evaluations, determine shift staffing requirements, and make personnel recommendations.
  • Report staffing shortages and absentee call-ins to the Nurse Supervisor.
  • Develop work assignments and provide leadership to nursing personnel on the assigned unit or shift.
  • Conduct daily rounds to ensure nursing personnel perform assignments according to nursing standards.
  • Meet regularly with nursing personnel to identify and correct problems and improve services.
  • Maintain effective working relationships with facility personnel and departments.
  • Receive and give nursing reports at the beginning and end of shift duty.
  • Report occupational exposures, fraud, safety issues, and suspected information-system violations according to facility procedures.
  • Inform nursing personnel about new admissions and prepare rooms for new residents.
  • Greet and orient newly admitted residents and family members.
  • Arrange diagnostic and therapeutic services ordered by physicians.
  • Consult with physicians regarding resident care, treatment, and rehabilitation.
  • Review resident charts for treatments, medication orders, diets, and other care requirements.
  • Ensure prescribed treatments are properly administered and evaluate residents’ physical and emotional status.
  • Provide or oversee direct nursing care according to resident care plans and wishes.
  • Coordinate social and activity programs with nursing schedules.
  • Notify physicians and next of kin about resident accidents, incidents, or changes in condition.
  • Carry out restorative and rehabilitative programs, including self-help and care.
  • Inspect treatment areas and resident care rooms to ensure they are clean and safe.
  • Provide professional nursing services including catheterization, tube feedings, suctioning, dressing changes, colostomy care, drainage-bag care, specimen collection, vital-sign measurement, range-of-motion exercises, and care of dying residents.
  • Use restraints when necessary and according to established procedures.
  • Monitor seriously ill residents and check residents who cannot call for help.
  • Check food brought by residents’ families or visitors against dietary allowances.
  • Arrange transportation and escorts for discharged residents.
  • Notify families of resident deaths, contact funeral homes when requested, and ensure postmortem procedures are followed.
  • Develop, plan, conduct, and schedule in-service training for nursing personnel.
  • Orient new employees to the shift, policies, procedures, job position, and duties.
  • Assist with clinical supervision and training of nurse aide trainees.
  • Attend required external, annual facility, advance-directive, and continuing-education programs.
  • Monitor compliance with safety regulations, dress codes, hand hygiene, infection-control procedures, and personal protective equipment requirements.
  • Ensure adequate personal protective equipment, medications, medical supplies, and equipment are available.
  • Maintain assigned work areas, resident rooms, treatment areas, and nursing equipment in a clean, safe, and sanitary condition.
  • Participate in infection-control and hazardous-condition reporting programs.
  • Ensure staff participate in fire-safety and disaster-preparedness drills.
  • Ensure personnel use lifting and safety equipment correctly.
  • Review resident care plans daily and report needed changes to the Nurse Supervisor.
  • Ensure nurses’ notes document compliance with care plans and treatments.
  • Review care plans for appropriate goals, problems, approaches, and revisions.
  • Ensure certified nursing assistants understand and follow resident care plans.
  • Assist with planning, scheduling, and revising the Minimum Data Set, Resident Assessment Protocols, and care-plan triggers.
  • Report suspected or known fraud involving false billings, cost reports, or kickbacks.
  • Provide requested data to the Quality Assurance and Assessment Committee.

PACS Group has appointed Carey P. Hendrickson as chief financial officer, effective 27 April 2026. Hendrickson brings nearly four decades of financial leadership across healthcare, senior living and media sectors. PACS Group has announced that co-founder and chief financial officer Mark Hancock will retire on 30 June 2026. He will continue serving on the company's board of directors as vice chairman. Hancock co-founded PACS in 2013 with Jason Murray, starting with two post-acute care facilities in San Diego. Dr Conway currently serves as CEO of Optum, UnitedHealth Group's health services division with revenues exceeding $200 billion.
