Requirements
  • Graduate of an accredited school of vocational or practical nursing.
  • Hold an active state license as a licensed vocational or practical nurse, or a multistate license issued by a Nurse Licensure Compact state.
  • Have one year of experience as a licensed vocational or practical nurse in a clinical care setting.
  • Have nursing skills consistent with generally accepted standards of practice.
  • Have reliable transportation and valid auto liability insurance.
  • Provide proof of current CPR certification.
Responsibilities
  • Assist in identifying the patient’s physical, psychosocial, and environmental needs under the direction of the registered nurse, as evidenced by documentation.
  • Be present within partnering communities and clients’ independent residences.
  • Promote the agency philosophy and administrative policies to ensure quality of care.
  • Communicate effectively with patients, families, team members, and other healthcare professionals through clinical notes, case conferences, and communication notes; complete appropriate documentation in a timely manner.
  • Monitor assigned cases to ensure compliance with third-party payer requirements.
  • Participate in planning and implementing care with the patient, representative if any, caregivers, and registered nurse; provide services ordered by the physician as indicated in the plan of care.
  • Provide care using infection control measures that protect staff and patients.
  • Provide patient and caregiver education.
  • Participate in agency-sponsored in-service training.
  • Participate in the agency’s quality assurance and performance improvement program.
  • Demonstrate professional growth and competency.
  • Ensure continuity of care through quality patient care.
Desired Qualifications
  • Home health experience is preferred.
  • Experience with the Homecare Homebase platform is a plus.
