Requirements
  • Current, unrestricted California Physical Therapist license.
  • Current BLS/CPR certification.
  • Valid California driver's license with a clean driving record.
  • Graduation from an accredited Physical Therapy program.
  • Ability to work effectively as part of an interdisciplinary team including registered nurses, occupational therapists, speech therapists, medical social workers, and home health aides.
  • Strong clinical judgment, verbal and written communication, time management, and problem-solving skills.
  • Ability to self-direct, manage time independently, and maintain professionalism in a fast-paced setting.
  • Commitment to annual compliance training and continuing education requirements.
  • Ability to participate in on-call or weekend coverage rotation as required.
Responsibilities
  • Provide hands-on therapeutic interventions including gait training, balance and neuromuscular re-education, transfer training, therapeutic exercises, manual therapy, and pain management techniques, as appropriate under the license and within a home health setting.
  • Assist in managing patients with assistive devices, prosthetics and orthotics, wheelchairs, and home safety equipment.
  • Conduct detailed patient evaluations and assessments in the home environment.
  • Implement individualized plans of care under the supervision and guidance of a Physical Therapist Case Manager.
  • Provide accurate, timely documentation of patient care within 24–48 hours, consistent with the plan of care and compliance standards.
  • Educate patients and family members or caregivers on functional mobility, exercise programs, safety techniques, and proper use of adaptive equipment.
  • Collaborate with the Physical Therapist Case Manager to prepare for and assist in patient treatments and updates to the care plan.
  • Participate in interdisciplinary coordination of services, including identifying needs for occupational therapy, speech therapy, home health aide, medical social worker, and registered nurse interventions, and report to the Physical Therapist Case Manager as appropriate.
  • Use equipment and supplies effectively to provide efficient care while maintaining cost awareness.
  • Provide timely, relevant updates to the supervising Physical Therapist Case Manager and participate in weekly interdisciplinary case conferences as required.
  • Ensure patient privacy and dignity in documentation and care delivery.
  • Meet productivity expectations as directed by the Physical Therapist Case Manager.
  • Participate in initial evaluations, reassessments, and discharge planning in collaboration with the Case Manager Physical Therapist.
  • Perform other duties as assigned by the supervising Physical Therapist Case Manager.
Desired Qualifications
  • One year of experience as a Physical Therapist is preferred; new graduates and internal candidates may be considered.
  • Prior home health experience is highly desirable.
  • Experience treating high-acuity patients is a plus.
  • Familiarity with case management principles is preferred.
  • Familiarity with electronic documentation and mobile healthcare tools.
  • Participation in professional development, clinical training, and skill-refinement opportunities.
  • Keeping clinical knowledge current and remaining informed about best practices and home health standards.
  • Using relevant communication for assigned patients to prioritize care effectively.
