Requirements
  • Sufficient education to qualify as a registered nurse.
  • A current registered nurse license issued by the state of Oregon.
  • Ability to pass a criminal history background check.
  • Knowledge, or the ability to acquire knowledge, of symptoms and challenges faced by people with mental illness and addictions.
  • Knowledge, or the ability to acquire knowledge, of relevant Oregon Administrative Rules.
  • Ability to communicate effectively verbally and in writing and complete responsibilities thoroughly and on time.
  • Ability to work professionally with the public and coordinate services across agencies and community groups.
  • Ability to develop cooperative and respectful relationships with clients and their families.
  • Knowledge, or the ability to acquire knowledge, of the recovery model.
  • Ability to represent the interests of the customer and agency favorably in the community.
  • Ability to work well with teams and other groups.
  • Ability to handle highly confidential information and adhere to client confidentiality rules and laws.
  • Functional knowledge of business English and medical terminology, or the ability to acquire it.
  • Good spelling and basic mathematical skills.
  • Ability to learn assigned tasks and follow general office procedures.
  • Good organizational and time management skills.
  • In-depth knowledge of standard office equipment.
  • Valid driver’s license and eligibility to be insured or personal auto insurance for client service purposes and travel.
  • Ability to pass criminal history check requirements under ORS 181.536-181.537 and OAR 410-007-0200 through 410-007-0380.
  • Pre-hire drug screen.
  • Personal auto insurance with at least $300,000 bodily injury liability coverage and $100,000 property damage liability coverage for vehicles used for CCS business.
  • Ability to meet the physical demands of the position, including standing, walking, stooping, kneeling, crouching, crawling, occasional climbing or balancing, and regularly lifting or moving up to 10 pounds.
Responsibilities
  • Administer oral medications and injections according to physician prescriptions and procedures.
  • Take and record patients’ pulse, temperature, respiration, and other general physical-condition measures.
  • Lead prescribed individual and group therapy or education sessions as part of therapeutic procedures.
  • Consult with community agencies and families to coordinate treatment.
  • Assist ACT participants with taking medications independently through behavior tailoring, coaching, reminders, and other strategies.
  • Issue medications from the dispensary and maintain required records.
  • Educate and train staff on medication administration and ensure documentation complies with policies.
  • Participate in joint case planning and coordination with agency employees and contractors.
  • Act as a liaison between clients and other agencies and advocate for clients when appropriate.
  • Maintain client records in an electronic medical record, including documentation of interactions, behavioral health assessments, and individual service plans.
  • Triage and assess clients for medical concerns and make referrals to hospitals and other agencies.
  • Formulate individualized nursing care plans and participate in patient care meetings.
  • Consult with other health team members and provide guidance as necessary.
  • Monitor patient conditions, note status changes, use clinical judgment, and take appropriate action.
  • Complete paperwork professionally, accurately, and promptly in accordance with Oregon Administrative Rules and other applicable guidelines.
  • Ensure required paperwork is present and client records are properly maintained.
  • Perform other duties as assigned.
