  • Ability to collaborate with an interdisciplinary crisis team, including BERT clinicians, nursing, and medical teams, and build and maintain working relationships with external community providers and programs.
Responsibilities

  • Serve as a behavioral health resource for patients and clinicians across the hospital, including BERT, Psychiatry Consultation-Liaison, Code CARE, and medical and surgical units; identify and connect patients to appropriate outpatient therapy, psychiatry and medication management, intensive outpatient and partial hospitalization programs, residential and inpatient long-term treatment programs, and other community behavioral health services.
  • Support hospital-based behavioral health clinicians in identifying appropriate next-level-of-care options and resources for patients.
  • Build and maintain relationships with community clinics and outpatient and residential providers to facilitate warm handoffs and streamline referrals; make and track referrals to these programs.
  • Collaborate with the Crisis Response Bridge Clinic, including referring appropriate patients and supporting ongoing communication and coordination between BERT and the clinic.
  • Document placement searches and transfer activities in the electronic health record and internal tracking tools according to program standards; support collection of bed-search and boarding-time metrics for quality improvement and reporting; provide feedback on system gaps and workflow improvements; participate in training and program evaluation activities.
Desired Qualifications

  • Bilingual ability.
  • Knowledge of the pediatric behavioral health continuum of care, including inpatient psychiatric admission criteria, intensive outpatient and partial hospitalization programs, residential treatment, outpatient therapy and psychiatry resources, and the regional landscape of receiving facilities and community providers.
  • Medical terminology knowledge sufficient to review laboratory results and clinical documentation.
  • Prior experience with inpatient psychiatric placement, bed-finding, or hospital-to-hospital transfer coordination.
