- A minimum of two years of higher education and experience working with the substance abuse or homeless population.
- An active Licensed Practical Nurse license is required.
- General counseling and communication skills are required.
- Computer skills are required, including knowledge of AWARDS, Microsoft Word, Microsoft Excel, and Microsoft Outlook.
- Interpret medical information supplied by providers.
- Coordinate medical services to facilitate intake physicals.
- Ensure that physical exams, laboratory work, and PPD status are updated for all participants.
- Check the medication log to ensure participants’ compliance with their medication regimen.
- Monitor the inventory of medical supplies and administer medication.
- Review the Prescription Monitoring Program system weekly and for all new admits.
- Review ePACES to determine participants’ medical entitlements.
- Perform other duties as assigned by the Program Director.
- Serve as a stand-in in the absence of the Program Director.
S:US helps New Yorkers who face homelessness, disability, behavioral health and addiction challenges, poverty, criminal justice involvement, and trauma access a wide range of supports across New York City and Long Island. It coordinates services such as case management, housing and stabilization, behavioral health treatment, recovery supports, rehabilitation, advocacy, and community-based programs tailored to each person’s needs. By linking housing, health, justice, and community resources, S:US delivers integrated, place-based services that address multiple barriers at once rather than focusing on a single issue. The goal is to help people lead purposeful lives, improve neighborhoods, and break the cycle of poverty for current and future generations of New Yorkers.
