Responsibilities

  • Maintain ongoing communication with potential clients and families
  • Coordinate various aspects of a client’s care
  • Document quality assurance & reassessment visits
  • Modify service plans to support clients' ongoing needs
  • Communicate effectively with clients, family members, colleagues, and referral providers
  • Support business growth goals through various methodologies
  • Adhere to company policies and procedures
