Requirements
  • A minimum high school diploma or equivalent or higher.
  • New York State Certified Peer Specialist preferred or completion of an internship or comparable training in peer advocacy.
  • Experience providing advocacy services to people who are mentally ill and/or homeless.
  • Past or current participation in behavioral health, substance use, or homeless services is preferred.
Responsibilities
  • Engage participants from the referring location and participate in transition planning, including assessing community transition supports needed to stabilize each participant.
  • Conduct individual sessions focused on prevention, independent community living, and optimal mental and emotional health, including family and social relationships, stress and symptom management, activities of daily living, medication management, and housing readiness.
  • Provide intensive emotional and practical support as participants transition into their communities and supportive housing or living environments.
  • Work with participants and housing providers to resolve clinical issues affecting participants’ ability to obtain, manage, and retain supportive housing.
  • Assist and instruct participants with daily living skills, including personal hygiene, grooming, laundry, nutrition and meal preparation, budgeting, socialization, and maintaining a safe living environment.
  • Build and maintain collaborative relationships with community providers and network members to connect or reconnect participants with services and supports.
  • Provide community navigation, including accompanying participants to their first behavioral health and medical appointments, travel training, reengagement in community care, and referrals to services while identifying and addressing service-access barriers.
  • Establish collaborative working relationships with referring treatment teams and other partners to plan appropriate participant discharges.
  • Advocate with and for participants to achieve personal goals and address barriers such as fear, stigma, and communication disparities with community providers and supports.
  • Provide required on-call coverage and work flexible hours.
