Responsibilities
Conduct ongoing health assessments and support the health needs of people in certified residential, day habilitation, and respite settings.
Respond to health concerns and medical emergencies with clinical guidance and support.
Partner with physicians, specialists, pharmacies, and medical liaisons to coordinate medical care and contribute to person-centered care planning.
Collaborate with medical liaisons to coordinate timely scheduling of appointments, hospitalizations, and related services, and ensure resulting care plans and medical recommendations are followed.
Develop, implement, and maintain individualized Plans of Nursing Service to coordinate nursing care and define responsibilities for direct support professionals.
Delegate nursing tasks to direct support professionals in accordance with the New York State Nurse Practice Act and monitor and evaluate their competency in health-related supports and delegated tasks.
Oversee medication management processes, including reviewing medication administration records and reconciliation, and address medication errors through investigation and follow-up.
Participate in incident management, including clinical review of reportable incidents.
Ensure all actions comply with OPWDD regulations, agency policies, and administrative memoranda.
Support agency survey preparation, including participation in on-site surveys and remediation of identified deficiencies.

Pay rate for this position: $73500 / yr - $88000 / year
The salary of the finalist selected for this role will be set based on a variety of factors, including but not limited to departmental budgets, qualifications, experience, education, licenses, specialty, and training.
