Requirements
  • A Master of Science in Nursing from an accredited school of nursing, or an equivalent qualification permitted by state law.
  • At least two years of Nurse Practitioner experience.
  • Understanding of palliative medicine principles, home health, hospice care eligibility, and Medicare and Medicaid regulations.
  • Familiarity with the interdisciplinary care process.
  • Ability to meet mandatory continuing education requirements for the employer and licensing boards.
  • Proficiency in point-of-care software and technology.
  • Ability to work autonomously in an advanced practice role.
  • A current registered nurse license and Nurse Practitioner certification or license as required by state law.
  • A collaborative agreement with a supervising physician, or willingness to establish one.
  • Part B Provider certification.
  • A federal Drug Enforcement Administration license and state Drug Enforcement Administration license if required.
  • A valid driver's license and current automobile insurance.
Responsibilities
  • Collaborate with the Palliative Medical Director and care team to deliver comprehensive, patient-centered palliative care.
  • Perform in-depth patient assessments, including physical, emotional, and psychosocial evaluations.
  • Develop and implement individualized care plans in partnership with patients and families.
  • Manage and adjust treatment plans based on patient needs and progress.
  • Prescribe medications, including controlled substances, and order diagnostic tests as permitted by state law.
  • Maintain timely and accurate clinical documentation in compliance with professional standards.
  • Communicate with physicians, caregivers, and other healthcare professionals.
  • Participate in the evening and weekend call rotation as needed.
  • Facilitate referrals for additional care services when appropriate.
Desired Qualifications
  • Specialty in Family Medicine or Adult Medicine.
  • Experience with Part B billing.
  • Advanced Certified Hospice and Palliative Nurse certification.
