Requirements
  • Licensed Nurse Practitioner.
  • Licensure in Florida, Michigan, Ohio, Virginia, or Washington, or willingness to obtain licensure in these states.
  • At least 3 years of clinical care experience as a Nurse Practitioner, ideally in primary care or an emergency department.
  • Experience working with and managing populations affected by behavioral health or substance use disorder conditions.
  • Experience in care management and coordination.
  • Willingness to become certified in psychiatric nursing at the registered nurse level.
  • Ability to use technology comfortably and perform data-driven care.
  • Ability to maintain professional boundaries and engagement with a population facing challenges and working in non-traditional conditions.
Responsibilities
  • Perform virtual whole-person clinical assessments, including behavioral and physical health histories and virtual physical examinations.
  • Treat conditions by prescribing medications on an ongoing and short-term basis.
  • Identify undiagnosed conditions and order additional studies when indicated to confirm or rule out diagnoses.
  • Collaborate with medical records and clinical documentation specialists, certified coders, and medical assistants.
  • Make individualized recommendations and referrals based on assessed needs.
  • Support individuals in developing self-management skills and provide person-centered care through motivational interviewing and holistic care planning.
  • Organize information to support appropriate clinical documentation and successful disability applications when appropriate.
  • Perform virtual unscheduled-care assessments and transition-of-care visits.
  • Provide urgent-care treatment for COPD exacerbations, uncontrolled diabetes, congestive heart failure exacerbations, cold and flu symptoms, urinary tract infections, pain, anxiety, depression, and suicidal ideation.
  • Conduct transition-of-care visits within 7 days of emergency department or hospital discharge.
  • Lead individual progress reviews in partnership with firsthand guides.
  • Assist with implementing new interventions and work processes.
  • Provide guidance and support to in-person Health Guides and firsthand guides.
Desired Qualifications
  • Psychiatric registered nurse certification is optional.
  • Primary care or emergency department experience is ideal.

firsthand raises $10M of $32M round after staff cuts following UnitedHealthcare contract loss

firsthand, a New York-based serious mental illness care provider, has secured $10 million of a planned $32 million funding round, according to public filings. The raise follows significant setbacks for the company, which was forced to cut most of its staff in March 2025 after losing a major Medicaid contract through UnitedHealthcare. Founded in 2021, firsthand uses a peer support model where individuals with lived experience of serious mental illness engage with patients at home. The company previously raised $28.1 million in its Series B from GV in February 2023. The new funding signals renewed growth momentum. firsthand has established a partnership with Carelon focusing on total cost of care for serious mental illness patients. To date, the company has raised approximately $52.9 million across three funding rounds.
