Requirements
  • A BA or BS in any field and at least 1 year of case management experience, or at least 2 years of case management experience in lieu of a BA.
  • Experience working with culturally diverse populations.
  • A valid California driver's license, proof of insurance, and a personal vehicle; ability to transport clients in a personal vehicle as needed.
  • Must not be under any type of legal supervision, including probation or parole.
  • Verification of employment eligibility and a background check.
  • Basic computer skills.
  • Ability to maintain active licenses and/or registrations as applicable, such as BBS or CCAPP.
  • Ability to work regular attendance schedules, including holiday or weekend work when required.
  • Ability to move about the worksite, including walking, standing, and climbing stairs or using a mobility device; drive, carry up to 20 pounds, listen, and speak as required.
  • Ability to synthesize information and make decisions without immediate supervision, manage distractions and changing priorities, interact professionally in high-stress situations, create and follow plans, and meet deadlines.
Responsibilities
  • Provide intensive, wraparound, field-based case management services to support clients’ recovery goals.
  • Actively engage clients through rapport-building, consistent contact, motivational interviewing, and outreach services.
  • Deliver services consistent with ethical standards and a harm reduction, strength-based, trauma-informed framework.
  • Develop and maintain knowledge of community resources; complete needs assessments, link clients to resources, monitor participation, facilitate problem solving, and accompany clients to linkage appointments when appropriate.
  • Teach prosocial and recovery skills, including communication, problem solving, and anger management, to support long-term community stability.
  • Engage family members, probation, court personnel, and other collateral contacts as appropriate; provide mental health recovery education and advocate for clients.
  • Conduct custody pickups from Los Angeles County jails and accompany clients to court appearances.
  • Apply a racial equity lens to all elements of clinical care.
  • Contribute to planning and executing clinical interventions in collaboration with the multidisciplinary treatment team.
  • Participate in coordinated outreach and engagement efforts with the treatment team.
  • Collaborate with external partners, including residential substance treatment facilities and primary care clinics, to ensure integrated care.
  • Maintain comprehensive case notes in the electronic health record according to program guidelines.
  • Follow Department of Health Services and Office of Diversion and Reentry documentation guidelines.
  • Participate in program data collection according to program guidelines.
  • Uphold the agency mission, values, policies, procedures, and principles.
  • Perform other duties assigned by the supervisor.
Desired Qualifications
  • Previous case management experience.
  • Lived experience with homelessness, mental health or substance use recovery, or the criminal justice system.
  • Spanish-speaking ability.
  • Experience working with forensic and/or dual-diagnosis populations.
  • Experience working on an interdisciplinary team in a mental health or hospital setting.
