Requirements
  • At least one year of administrative experience or an equivalent combination of education, training, or experience.
  • A high school diploma or GED is required.
Responsibilities
  • Coordinate departmental office or technical support functions, including organizing clerical or technical support functions and prioritizing tasks to ensure efficient departmental operations.
  • Use the Action List to track referrals for radiologic testing and specialty consultation and remove barriers to accessing primary and specialty services.
  • Determine and secure required resources and support services.
  • Contact patients identified through the population health dashboard Action List based on required clinical core measures.
  • Prepare and generate routine reports and responses at the request of the BLISS CEO, determine the information required to complete reports, contact appropriate information sources, compile data, and prepare statistical information.
  • Maintain and update databases and conduct periodic audits to ensure data integrity.
  • Participate in the Pediatric Birthday Card Initiative.
  • Complete tasks assigned by the BLISS CEO.
  • Generate reports of vaccinated and non-vaccinated patients and participate in the Vaccine Outreach and Counseling Program.
  • Participate in training activities organized for population health, the Action List, and other training requested by the BLISS CEO.
  • Arrange information technology services, including repairs, setups, hardware upgrades, email, and internet connections, through Change Request.
  • Prepare reports at the request of the BLISS CEO.
  • Participate in the Primary Care Physician Panel Growth Initiative.
  • Follow the BronxCare Diagnostic Test Tracking Policy PM.DT10.
  • Review all recall alerts from providers.
