  • Communicate with patients, families, clinical staff, and non-clinical staff; identify barriers to appointment compliance and insurance approval; and track assistance provided.

  • Escalate case management when medical assessment is needed and prioritize referral requests using medical protocols, expediting urgent requests.
  • Request and coordinate team and patient meetings as needed or requested by patients, and participate as an active care-team member.
  • Obtain insurance authorization for visits and applicable testing, attach missing referral records, and document referral and authorization communications in the electronic health record.

  • Communicate medical information to insurers and coordinate peer-to-peer reviews for denied services.
  • Prepare authorization requests using prior insurer decisions, specialty and subspecialty knowledge, medical terminology, International Classification of Diseases codes, Current Procedural Terminology codes, insurance policies, permissible requests, relevant treatment history, and appropriate descriptions of unsuccessful treatments.

  • Work with providers and clinical staff to establish the best care plan for patients.
  • Process outgoing referrals, discuss outside University of Rochester Medical Center care options with patients, and ensure Meaningful Use requirements are met.
  • Ensure the Summary of Care is transferred electronically through Epic to the referred-to office; when Epic transfer is not possible, send the information by fax or mail.
  • Process incoming referrals originating outside the University system, enter them into the electronic health record according to approved protocols, coordinate ancillary testing, and obtain outside records needed for appointments.
