Requirements
  • The candidate must be a Registered Nurse.
  • The candidate must have at least one year of RN experience in acute care hospital, oncology, long-term care, hospice, or home health; school internships and clinical hours may be considered in individual cases.
  • The candidate must hold a current license to practice nursing in the state of employment.
  • The candidate must hold CPR certification.
  • The candidate must have current automobile insurance and a valid driver’s license.
  • The candidate must understand terminally ill patients and their families, hospice services, the roles of disciplines providing hospice services, community resources, general nursing practice, managed care principles, regulatory guidelines including Medicare, Medicaid, and ACHC, and human resource management principles.
  • The candidate must have patient assessment, documentation, program management, and nursing assessment tool skills.
  • The candidate must maintain transportation for work and be able to operate nursing assessment tools, computers, facsimile equipment, a copier, and a cell phone or beeper.
  • The candidate must work effectively with patients and families from all socioeconomic backgrounds and demonstrate flexibility, tact, maturity, cooperation, and composure in a safety-sensitive environment.
Responsibilities
  • Serve as the primary point of contact for patients and families throughout the hospice journey.
  • Deliver and document skilled nursing care according to individualized plans of care in patients’ homes, assisted living facilities, or other settings.
  • Perform comprehensive initial, ongoing, in-person, and telephonic patient assessments and reassessments.
  • Deliver skilled nursing interventions and symptom management aligned with goals of care.
  • Develop and update individualized plans of care, including visit frequency and risk identification.
  • Coordinate care with physicians, nurse practitioners, licensed practical nurses, certified nursing assistants, social workers, chaplains, and other interdisciplinary team members.
  • Provide emotional, physical, and spiritual support, education, comfort, and bereavement guidance to patients and their loved ones.
  • Teach patients, family members, and caregivers about disease processes, self-care, end-of-life care, medications, nutrition, dietary needs, symptom and pain management, hospice philosophy and services, and end-of-life expectations.
  • Communicate changes in condition and signs or symptoms of decline to appropriate providers, including nurse practitioners, primary care providers, and hospice physicians.
  • Support home health, palliative, and hospice referrals and transitions between levels of care.
  • Maintain timely and accurate clinical records, including screening and assessment findings, physician orders, progress notes, and care plans.
  • Maintain effective working relationships with the interdisciplinary group and involve the team when the patient’s care plan warrants participation.
  • Follow infection-control, regulatory, policy, and compliance standards.
  • Participate in quality-improvement, scheduling, on-call, patient-support, and interdisciplinary-group activities.
