  • The candidate must be licensed by the New York State Department of Education.
  • The candidate must have experience in OPWDD-certified homes, community residences, and nursing homes.
  • First Aid and CPR certifications are preferred.
  • The candidate must be available for on-call coverage by phone and Lotus Notes access 24/7.
  • A valid driver's license is preferred.
Responsibilities
  • Manage the nursing care of individuals, including determining the need for immediate physician attention, executing physician or dentist medical orders, and coordinating nursing care with routine facility activities and comprehensive treatment plans.
  • Provide overall medical, dental, and nutritional care for OPWDD individuals residing in the facility while adhering to applicable regulations and reviewing resident progress.
  • Complete pre-admission evaluation studies and plans.
  • Evaluate each individual's need for nursing care on a periodic and continuing basis and provide care when appropriate under the supervision of the Nursing Manager and QIDP.
  • Complete the evaluation study, program design, and client placement at admission to the residence.
  • Conduct periodic reevaluations of the type, extent, and quality of services and programming.
  • Develop discharge plans and refer individuals to appropriate community resources.
  • Conduct in-service training regarding personal hygiene, medication administration, family life, and sex education.
  • Control communicable diseases and infections through identification, assessment, and reporting to medical authorities.
  • Implement appropriate protective and preventive measures.
  • Develop treatment plans with the Interdisciplinary Team by evaluating nursing needs and selecting nursing procedures and practices that best serve individual health care needs.
  • Record nursing observations and actions in individuals' medical records, including nursing care, medications and treatments, treatment reactions, tests, fluid and solid intake and output, vital signs, and nursing needs.
  • Supervise medication administration practicum and sign off monthly on medication administration to ensure outdated and discontinued drugs are properly disposed of.
  • Be available for on-call duty and varied shifts as needed.
  • Coordinate with consulting and contracted medical services to ensure recommended treatments are carried out, integrated, and documented in treatment plans.
  • Maintain inventory standards for medicines, solutions, supplies, and equipment.
  • Account for the issuance and inventory of controlled drugs and narcotics.
  • Provide individuals and families with information about preventive health care, personal hygiene, family or self-treatments, self-medication, and possible medication side effects.
  • Serve as a liaison, interpreter, or accompaniment for individuals receiving services from outside health care providers.
  • Train direct support professional staff through demonstrations and written and oral instructions on nursing techniques and nursing equipment.
  • Carry out agency management policies in accordance with OPWDD and other funding agency guidelines.
