About the job
- Maintain active California Nurse Practitioner and Registered Nurse licenses, an active furnishing license, National Provider Identifier, Drug Enforcement Administration registration, and Basic Life Support certification.
- Demonstrate proficiency with electronic medical records.
- Provide routine and urgent assessments and manage chronic diseases.
- Conduct post-discharge follow-ups, medication reconciliation, and laboratory and diagnostic work.
- Coordinate care with facility staff, specialists, families, and powers of attorney.
- Complete same-day documentation and comply with Centers for Medicare & Medicaid Services and regulatory requirements.
- Deliver care three days per week, including on-site care across Stockton, Modesto, and Amador and telehealth on other days.
- Geriatrics or facility-based care experience is preferred.
