Requirements
  • Six months to one year of experience in a related field is desired.
  • A current Nursing Assistant License in the State of New Hampshire is required.
  • A driver's license is required to travel throughout New Hampshire for home health care and other job-related activities.
  • Automobile insurance coverage must be maintained at the established EHS minimum throughout employment.
  • The work schedule may include 8 to 12 hours per day, including weekend, evening, night, and on-call coverage; staff must cover absences by rotating or working extra time.
Responsibilities
  • Perform delegated nursing, homemaking, and other delegated duties for patients or clients of all ages and their families in their homes or other designated facilities.
  • Perform PT/INR checks in the home as ordered by a physician and report results according to policy.
  • Report and document patient or client needs and responses to the appropriate Case Manager or Clinical Manager.
  • Immediately report changes in patient or client condition or situations.
  • Follow the patient or client Home Health Aide care plan established by the primary nurse or therapist while maintaining a healthy, safe environment.
  • Reinforce teaching from the primary nurse or therapist by reviewing personal care techniques with caregivers and patients.
  • Complete documentation in clinical records, timesheets, and quality-control logs.
  • Attend a minimum of 12 hours of in-service training per year.
  • Provide care while promoting patient or client dignity and maintaining a calm, supportive, and therapeutic environment for clients, patients, and their families.
Desired Qualifications
  • A high school diploma or equivalent is preferred.
  • Six months to one year of experience in a related field is desired.
