Requirements
  • A bachelor's degree in a related field and at least two years of experience in the healthcare or nonprofit field.
  • Alternatively, a high school diploma or equivalent and at least seven years of experience in the healthcare or nonprofit field.
  • Willingness to travel extensively throughout all boroughs.
  • Ability to work a flexible schedule, including non-office hours, evenings, weekends, and holidays when necessary.
  • Availability to be on call during non-office hours, evenings, weekends, and holidays when necessary.
  • Ability to communicate effectively and demonstrate professional interpersonal skills.
  • Ability to work independently, manage competing priorities, exercise sound judgment, and solve problems with minimal supervision.
  • Knowledge of incident management functions and relevant regulatory requirements.
  • Ability to produce clear, concise, accurate, and logically organized written communications.
  • Ability to comply with HIPAA and other applicable regulatory requirements.
Responsibilities
  • Serve as the primary quality-assurance contact for program staff and external oversight bodies regarding incident-management reporting and processes.
  • Interpret and apply incident-management regulations and provide technical support to staff regarding those regulations.
  • Triage incident reports to ensure protective measures are in place for consumer safety and attend daily triage meetings.
  • Review notifications, preliminary investigations, occurrence reports, and supporting documentation, and follow up with program staff regarding missing information.
  • Serve as the Quality Assurance co-chair in monthly Incident Review Committee meetings.
  • Complete minutes and follow-ups for Incident Review Committee meetings and submit them to program management and appropriate staff.
  • Review incidents to ensure Incident Review Committee minutes and follow-ups are completed and submitted for review in a timely manner.
  • Conduct incident-management training for staff at all levels.
  • Help develop and update incident-management training materials.
  • Maintain incident-management reporting logs and data supporting incident trends.
  • Manage data, including corrective action plan submissions, in required state computerized incident-management systems such as IRMA and NIMRS.
  • Draft monthly incident-trend reports, analyze trends involving consumer incidents, and request appropriate corrective actions.
  • Conduct post-incident analyses, document incident root causes accurately, and identify and track preventive actions.
  • Provide verbal and written incident updates to stakeholders and leadership.
  • Request additional information from external sources to thoroughly review incidents.
  • Provide required notifications and updates on incidents to department heads.
  • Coordinate with program leadership and Human Resources to support swift incident resolution.
