Requirements
  • The candidate must be a Registered Nurse with at least one year of RN experience in acute care hospital, oncology, long-term care, hospice, or home health; school internships and clinical hours may be considered in individual cases.
  • The candidate must hold a current license to practice nursing in the state of employment.
  • The candidate must hold CPR certification.
  • The candidate must have current automobile insurance, a valid driver’s license, and transportation for work.
  • The candidate must have knowledge of terminally ill patients and their families, hospice services, the roles of hospice disciplines, community resources, general nursing practice, managed care principles, and regulatory guidelines including Medicare, Medicaid, and ACHC.
  • The candidate must be able to operate nursing assessment tools, computers, facsimile equipment, copiers, and cell phones or beepers.
  • The candidate must be able to work in patients’ homes or institutional settings and tolerate potential exposure to infections, communicable diseases, odors, blood, excreta, and hazardous materials.
  • The candidate must support patients and families experiencing life-and-death issues and communicate effectively with people from all socioeconomic backgrounds.
Responsibilities
  • Serve as the primary point of contact for patients and families throughout the hospice journey.
  • Deliver and document skilled hands-on nursing care according to each patient’s individualized plan of care in the patient’s home, assisted living facility, or other setting.
  • Perform comprehensive initial, ongoing, in-person, and telephonic patient assessments and reassessments.
  • Deliver skilled nursing interventions and symptom management aligned with goals of care.
  • Develop and update individualized plans of care, including visit frequency and risk identification.
  • Collaborate with physicians, nurse practitioners, licensed practical nurses, certified nursing assistants, social workers, chaplains, and other interdisciplinary team members to coordinate care.
  • Provide emotional, physical, and spiritual support, education, comfort, and bereavement guidance to patients and loved ones.
  • Teach patients, family members, and caregivers about disease processes, self-care, end-of-life care, medications, nutrition, dietary needs, and safe caregiving techniques.
  • Communicate changes in patient condition or signs and symptoms of decline to the appropriate provider.
  • Support continuity of care by recognizing and supporting home health, palliative, and hospice referrals and transitions between levels of care.
  • Maintain timely and accurate clinical records, including screening and assessment findings, physician orders, progress notes, and care plans.
  • Follow infection-control, regulatory, compliance, and organizational policy standards.
  • Maintain effective working relationships with the interdisciplinary group and involve the team when the care plan warrants participation.
  • Participate in quality improvement, scheduling, on-call, and patient-support activities as applicable.
