Requirements
  • Graduate of an accredited school of nursing is required.
  • Three years of healthcare or medical experience is required.
Responsibilities
  • Provide individualized care coordination to patients and families, including timely diagnostic work-up scheduling, results reporting, communication with the multidisciplinary team, referrals, and follow-up.
  • Interact with co-workers, visitors, and other staff consistent with Jefferson values.
  • Collaborate with chronically ill or high-risk patients, caregivers, providers, staff, and community resources in a team approach.
  • Evaluate the needs of cancer patients and educate them about their cancer diagnosis.
  • Prepare patients and caregivers for what to expect throughout the cancer care continuum.
  • Create and promote adherence to care plans developed with patients, primary care providers, oncologists, and caregivers.
  • Provide clinical support to Patient Navigators and Sidney Kimmel Cancer Center patients.
  • Educate patients and families about disease processes and potential side effects.
  • Assist patients with treatment decision-making and develop and use decision aids as appropriate.
  • Attend and support tumor boards.
  • Reduce emergency room utilization and hospital readmissions through outpatient symptom management.
  • Follow patients throughout the care continuum, including inpatient admissions, in collaboration with inpatient care management resources.
  • Support transitions of care and coordination between Sidney Kimmel Cancer Center sites.
  • Provide clinical consultation to Patient Navigators.
  • Educate multidisciplinary colleagues about the roles of Nurse and Patient Navigators, identified barriers, available resources, and the navigation process.
  • Support program growth and patient experience in all patient interactions.
Desired Qualifications
  • Bachelor of Science in Nursing degree or completion of a Bachelor of Science in Nursing within three years of hire.
  • Experience in oncology or cancer care.
