Requirements
  • A high school diploma or equivalent is required.
  • Current and active state-approved medication aide or technician certification is required.
  • A basic understanding of Microsoft Office Suite, including Word, Excel, Outlook, and PowerPoint, is required.
  • Ability to walk or stand for extended periods and move throughout the community, resident apartments, and other areas is required.
  • Ability to occasionally stoop, kneel, crouch, crawl, climb, or balance is required.
  • Ability to communicate effectively and identify and interpret written information is required.
  • Ability to lift up to 50 pounds independently and up to 200 pounds with assistance is required.
  • Compliance with infection prevention and control protocols, including using personal protective equipment when necessary, is required.
  • Current training and ongoing education, and participation in continuous learning to enhance skills and knowledge, are required.
  • A criminal record check is required as a condition of employment.
  • Successful completion of a background check and drug screening, participation in required health assessments such as TB testing or physical evaluations, and use of designated personal protective equipment as required by company policy and law are conditions of employment.
Responsibilities
  • Provide direct resident care, including distributing medication according to each resident’s individualized plan of care.
  • Observe residents, communicate changes in condition to physicians, health care proxies, and supervisors, and provide timely interventions and communication as instructed.
  • Document relevant observations, red flags, incidents, interventions, communications, clinical procedures, and daily medication assistance and care throughout the shift.
  • Pass required information to the next shift and direct supervisor in a timely manner, and oversee Resident Assistants’ timely documentation to support quality resident care.
  • Check and record vital signs, weights, and blood sugars as assigned.
  • Use proactive interventions, monitoring, and safety practices during room checks and rounds.
  • Administer and document medications according to the five rights: right medication, route, resident, time, and dose.
  • Provide glucose monitoring and administer insulin when insulin-certified.
  • Assist residents with activities of daily living, including scheduled bathing or showering, oral care, toileting, and dressing.
  • Assist and monitor residents in meeting nutritional and hydration needs and document this in the nursing record.
  • Perform restorative nursing techniques, including proper positioning, activities-of-daily-living training, and passive range of motion.
  • Assist residents with transfers and ambulation as directed, use proper body mechanics, and respond to urgent calls as needed.
  • Encourage residents to participate in scheduled facility activities and assist them in getting to activities as needed.
  • Maintain a restraint-free environment and support fall-prevention programs for residents who need them.
  • Welcome, orient, and transfer residents according to facility procedures.
  • Assist with post-mortem care as instructed.
  • Help oversee the medication delivery system, including timely order entry, receipt and tracking, daily medication administration record documentation, narcotic counts, and medication check-in, to support residents’ medication regimens.
  • Notify the pharmacy and supervisor of medication issues and seek timely resolution.
  • Attend in-service classes and staff meetings, assist with orienting new employees, and lead and coach resident assistants and certified nursing assistants to improve performance.
  • Promote each resident’s physical, personal, and emotional well-being and support their return to their maximum level of self-care and independence when possible.
  • Understand residents’ advance directives and follow their wishes.
  • Ensure community staff communicate effectively with residents’ families about residents’ physical, personal, and emotional well-being and care.
  • Take direction from management, take initiative, and engage in conflict intervention and resolution for residents, families, employees, and others as needed.
  • Participate in staff activities that foster teamwork, unity, and morale.
  • Perform other duties as assigned.
Desired Qualifications
  • Insulin certification is preferred.
  • CPR or BLS certification is preferred.
