Requirements
  • A bachelor's degree in a human services-related field such as Psychology, Sociology, Special Education, Rehabilitation Counseling, Social Work, Communicative Disorders, Recreation Therapy, Criminal Justice or Criminology, or qualification as a Registered Nurse is required.
  • At least one year of experience working with people with developmental disabilities is required.
  • Must be approved by the Illinois Department of Human Services to meet the Qualified Intellectual Disability Professional requirement.
  • Must successfully complete company-provided CPR, First Aid, Crisis Prevention Institute, Qualified Intellectual Disability Professional, and Direct Support Person training within the specified timeframe; CPR, First Aid, and Crisis Prevention Institute training must be renewed annually.
  • Must be proficient with Microsoft Word, Excel, and Outlook.
  • Must pass state and federal registry checks, a criminal background check, and the DCFS Abuse and Neglect Tracking System check.
  • Must have a valid driver's license, maintain auto insurance, pass a driving history check and company driving criteria, and be at least 21 years old to drive a company vehicle.
  • Must be able to use a personal insured vehicle for company-related travel.
  • Must be able to perform the stated physical duties, including standing, walking, bending, squatting, reaching, twisting, CPR kneeling, and lifting or carrying up to 50 pounds.
Responsibilities
  • Provide case management and person-centered services for individuals with disabilities in a community-based setting.
  • Communicate with clients about progress toward goals and program success, including vocational, personal, social, guardianship, and advocacy needs.
  • Coordinate assistance from staff, family members or guardians, residential providers, and other service providers.
  • Assist with program needs such as lunch duty, medication pass, bus calls, and bus monitoring.
  • Develop person-centered client programming, evaluate progress, and maintain supporting documentation.
  • Establish implementation strategies, goals, and objectives with clients and applicable family members, guardians, residential providers, and other stakeholders.
  • Write new person-centered plans and update existing plans.
  • Monitor progress and document behaviors, incidents, and service activity.
  • Review staff documentation and file plans, reports, and forms in case files and Quantum according to procedures.
  • Educate clients, families, guardians, providers, and other service providers about maintaining benefits as applicable.
  • Ensure documentation is maintained according to regulatory specifications.
  • Supervise facility-based and community-based services, training, and integration opportunities.
  • Assist with developing community integration opportunities according to each client's person-centered plan.
  • Coordinate assignments of appropriate work, training, or activities for clients and ensure proper training, supervision, and transportation when needed.
  • Provide coverage for absent staff and assist with client needs.
  • Transport clients for community outings, employment, or appointments using a company vehicle or personal insured vehicle.
  • Assist with hiring, training, and empowering team members.
  • Communicate job expectations and evaluate team performance for continual improvement, efficiency, and effectiveness.
  • Initiate, coordinate, and enforce systems, policies, and procedures.
  • Mentor and develop team members through training, problem resolution, and performance evaluation.
  • Communicate regularly with staff about client updates and workplace changes.
  • Build and support an effective company culture aligned with company values and organizational goals.
