Capital Health launches program to provide hospital-level care at patients' homes. Capital Health has launched a new program that delivers hospital-level care to patients in their own homes, according to an Aug. 12 announcement. The Hospital at Home Program is designed to manage capacity at Capital Health hospitals by allowing eligible patients to receive care outside the traditional hospital setting. The program offers personalized treatment through daily virtual visits with a doctor and in-person nurse check-ins, as well as services such as physical therapy, lab work, and nutrition support. These services are coordinated by a multidisciplinary team. Participants must be New Jersey residents aged 18 or older, live within a 30-minute drive of Capital Health Medical Center - Hopewell, and meet specific clinical and safety criteria for eligibility. "The innovative care model for this program allows eligible patients to receive hospital-level diagnostics, treatments, and daily clinician visits in the comfort of their own homes," said Al Maghazehe, president and CEO of Capital Health. "Supported by our clinical teams and enhanced remote monitoring technology, Hospital at Home isn't just transforming how we provide care. More importantly, it means better outcomes for patients with fewer complications, faster recovery, and lower overall costs of care." Patients who may qualify include those with conditions such as pneumonia, chronic obstructive pulmonary disease (COPD), congestive heart failure (CHF), cellulitis, or other acute but stable diagnoses that can be managed safely at home. After diagnosis and initial treatment at Capital Health Medical Center - Hopewell, interested patients are assessed by a clinical review team before being admitted into the home-based program. Participants receive daily video visits from physicians affiliated with Capital Health Medical Group and twice-daily in-person nurse visits. The care team also includes pharmacists, therapists, nutritionists, social workers, case managers, and other specialists who help coordinate all aspects of patient support.
