Requirements
  • An Associate Degree or equivalent in Physical Therapy from an approved curriculum is required.
  • A current license for physical therapy assisting in the state where the employee practices is required.
  • The employee must have the ability to speak, read, write, and understand English.
  • The employee must have working knowledge of Windows, Microsoft Office Suite, and the World Wide Web.
  • The employee must have good written and verbal communication skills.
  • The employee must have good communication, comprehension, and interpersonal skills.
  • Continuous travel is required for Home and Community Services staff.
Responsibilities
  • Treat residents or clients with injuries, illnesses, or disabilities through therapeutic activities ordered by a physician and in accordance with Ohio physical therapy regulations.
  • Practice under the supervision of a Physical Therapist and communicate resident or client progress and status to the supervising therapist in a timely manner.
  • Develop and evaluate plans of care in partnership with patients, patient representatives when applicable, and caregivers.
  • Provide patient and caregiver education and counseling.
  • Communicate with physicians and other health care practitioners involved in the plan of care.
  • Screen residents or clients in accordance with facility or agency policy.
  • Complete all necessary documentation accurately and on time.
  • Assist with marketing and program development as requested.
  • Maintain productivity levels that meet facility or agency goals.
  • Maintain knowledge of the physical therapy field through related in-service and continuing education programs.
  • Assist the facility or agency in meeting standards required for state, federal, and other appropriate certifications.
  • Plan and complete in-services for facility or agency staff education as requested by management.
  • Attend all facility or agency meetings and in-services.
  • Follow applicable organizational policies, procedures, codes, regulations, and laws.
  • Provide care respectfully and protect resident or client dignity.
  • Report resident or family complaints and allegations of abuse, misappropriation, or other compliance concerns to appropriate supervisors.
  • Complete annual education requirements, including Compliance, Privacy, Security, and HIPAA training.
  • Perform other duties as assigned.

  • Two to four years of experience is preferred.
