Requirements
  • A current New York State registered nurse license is required.
  • An Associate's degree is required.
  • A minimum of three years of clinical experience in an assigned service is required.
  • Recent experience in case management, utilization management, discharge planning, or home care in a high-volume acute-care hospital is preferred.
  • PRI and case management certification are preferred.
  • Knowledge of computer systems applicable to the utilization review process is required.
  • Knowledge of MCG criteria and the ability to implement and use it are required.
  • Understanding of inpatient versus outpatient surgery, ICD-10 coding, and observation-status qualifications is required, with ICD-10 coding and observation-status qualifications preferred.
  • The ability to work independently and demonstrate organizational and time-management skills is required.
  • Strong analytical, data-management, and personal-computer skills are required.
  • Working knowledge of Medicare regulatory requirements and managed-care plans is required.
Responsibilities
  • Complete utilization management and quality screening for assigned patients.
  • Apply MCG criteria to monitor the appropriateness of admissions and continued stays, and document findings according to departmental standards.
  • Identify areas for clinical documentation improvement during utilization review and contact the appropriate department.
  • Identify at-risk populations using an approved screening tool and follow established reporting procedures.
  • Monitor length of stay and ancillary resource use on an ongoing basis, and take action to achieve continuous improvement in both areas.
  • Refer cases and issues to the Medical Director and Triad Team in compliance with department procedures, and follow up as indicated.
  • Communicate covered-day reimbursement certification for assigned patients.
  • Discuss payer criteria and issues with clinical staff on a case-by-case basis, and follow up with payers to resolve problems as needed.
  • Use quality screens to identify potential issues and forward information to the Quality Department.
  • Demonstrate proper use of MCG and documentation requirements through case review and inter-rater reliability studies.
  • Facilitate the removal of delays, document delays when they exist, and report internal and external delays to the Triad Team.
  • Collaborate with the healthcare team and appropriate departments to manage care across the continuum by maintaining communication with the Triad Team and healthcare team.
Desired Qualifications
  • A Bachelor's degree is preferred.
  • Recent experience in case management, utilization management, discharge planning, or home care in a high-volume acute-care hospital is preferred.
  • PRI certification and case management certification are preferred.
  • ICD-10 coding and observation-status qualifications are preferred.
