Requirements
  • High school diploma or General Educational Development (GED) credential is required.
  • Must meet Center for Medicaid and Medicare Services-approved state and/or national certification requirements within the required state or CMS timeline.
  • Must hold all appropriate state licensure, education, and training required for the role.
  • Must complete the organization dialysis training program successfully for continued employment.
  • Must complete CPR certification successfully for continued employment.
  • Must be able to perform direct patient care involving heavy lifting, moving patients, assisting with ambulation, prolonged standing, bending, and occasional pushing or pulling of equipment.
  • Must complete the Ishihara color blindness test as a condition of employment; reasonable accommodations may be considered if the test is not passed.
  • Must be able to travel between assigned facilities and various locations within the community; travel to regional, Business Unit, and corporate meetings may be required.
Responsibilities
  • Provide safe and effective dialysis therapy as part of the dialysis health care team under the direct supervision of a licensed nurse and in compliance with organizational policies, procedures, training, and applicable regulations.
  • Set up and operate hemodialysis machines.
  • Assist with maintaining a safe and clean working environment.
  • Support the Quality Enhancement Program and Continuous Quality Improvement activities, including patient satisfaction and process improvement activities.
  • Assist health care team members with patient-specific education regarding adequacy measures, disease processes, and vascular access.
  • Welcome assigned patients and inquire about their wellbeing since their last treatment.
  • Evaluate vascular access for patency, perform vascular access cannulation, and administer heparin when delegated or permitted by state law.
  • Obtain pre- and post-treatment vital signs and weights and perform pre-treatment vascular access evaluations.
  • Initiate dialysis treatments according to prescribed orders, including blood flow and dialysate flow.
  • Evaluate intradialytic problems and provide prescribed or directed interventions.
  • Monitor patients' responses to dialysis therapy.
  • Obtain vital signs before reinfusion and perform and document all functions required to discontinue treatment.
  • Discontinue dialysis treatment according to established procedures and evaluate patients before terminating venous access.
  • Obtain hemostasis and apply appropriate dressings.
  • Evaluate patients before discharge.
  • Perform and record pre- and post-dialysis evaluations, weights, and vital signs with initial identification.
  • Calculate and enter individual dialysis machine programming for ultrafiltration goals, treatment time, and ultrafiltration modeling as prescribed.
  • Ensure appropriate safety alarms are enabled, including Narrow Venous Limits 160.
  • Record accurate and timely vital signs and treatment parameters on the Hemodialysis Treatment Sheet with initial identification.
  • Complete accurate documentation related to patient treatment, including Hemodialysis Treatment Sheets and progress notes when applicable.
  • Report significant information and changes in patient condition directly to the registered nurse or supervisor.
  • Observe patients and conduct machine safety checks according to facility policy, reporting changes or unusual findings to the registered nurse or supervisor.
  • Perform and document interventions for unusual patient status and document patients' responses.
  • Evaluate vascular access during treatment, including arterial and venous monitoring pressures; provide interventions, document findings, and report unusual findings to the registered nurse or supervisor.
  • Obtain prescribed laboratory testing and prepare specimens for collection.
  • Clean blood spills immediately with appropriate disinfectant according to facility policy.
  • Prepare, organize, and efficiently use supplies and equipment to prevent wastage.
  • Monitor patients performing self-care under registered nurse supervision.
  • Assist in training applicable direct patient care staff on appropriate programs under nurse supervision.
  • Follow organization business policies, procedures, systems, and ethical business practices incorporated into training.
  • Maintain environmental integrity and aesthetics and ensure all areas are safe and clean.
  • Clean and disinfect dialysis machines, chairs, equipment, and surrounding areas between treatments according to facility policy.
  • Prepare for dialysis treatments, document as required, and perform pretreatment dialysis machine alarm testing, including Pressure Holding Testing.
  • Initiate the Solution Delivery System.
  • Complete and document ongoing participation in Continuous Quality Improvement activities.
  • Enter treatment data into the designated clinical application accurately and promptly.
  • Review treatment sheets for completeness, verify nursing signatures, and ensure omitted entries are completed or corrected by appropriate staff.
  • Prepare initial patient charts for admission and compile paperwork for placement in charts.
  • Prepare and print laboratory requisitions for prescribed or stat laboratory specimens according to laboratory destination.
  • Collect, label, prepare, and store laboratory samples according to laboratory specifications.
  • Ensure laboratory specimens are collected by the appropriate laboratory courier.
  • Assist with collecting information for infection control audits.
  • Support staff and patient adherence to infection control practices.
  • Follow infection control policies and procedures.
  • Participate in and reinforce infection control education for patients and families under the direction of the Clinical Manager or designee, as permitted by state law.
  • Follow newly admitted patients through their first four weeks of dialysis and coordinate their transitional dialysis care.
  • Reinforce education and care related to new patients as permitted by state law.
  • Liaise with appropriate staff members to provide the best patient experience and ensure all disciplines are scheduled to see patients according to plan.
  • Assist patients in understanding home dialysis products, the benefits of home dialysis, and how home dialysis can work for them.
  • Set up and perform treatments on all machines used in the Transitional Care Unit.
  • Assist patients in understanding treatment options, including demonstrating peritoneal dialysis setup.
  • Assist with coordinating transitions to patients' chosen dialysis modalities.
  • Assist with supply ordering and inventory for the Transitional Care Unit.
