Requirements
  • One year of recent nursing experience; this requirement may be waived for internal candidates and/or new graduates.
  • Graduation from an accredited school of nursing with a bachelor's degree.
  • A current California Registered Nurse license.
  • A current Basic Life Support card.
  • A valid California driver's license with an acceptable driving record.
  • All licenses and certifications must be current at the time of hire and maintained throughout employment.
  • Self-direction, critical thinking, organization, computer literacy, observation, verbal and written communication, problem-solving, basic math, and nursing skills demonstrated through the competency checklist.
  • Completion of annual compliance training and maintenance of required certifications.
Responsibilities
  • Provide hands-on patient care within the scope of the Registered Nurse license, including PICC lines, wound vacs and wound care, kangaroo and feeding pumps, nasogastric, gastrostomy, and jejunostomy tubes, staple and suture removal, JP, biliary, and nephrostomy drains, Foley and suprapubic catheters, PleurX drainage systems, tracheostomy care, blood draws, ostomy management, subcutaneous and intramuscular injections, intravenous antibiotic and total parenteral nutrition administration, and compression wraps.
  • Provide direct patient care as defined in the state Nurse Practice Act.
  • Implement a plan of care initiated and directed by the Case Management Registered Nurse.
  • Provide accurate and timely documentation within 24 to 48 hours consistent with the plan of care.
  • Assess patients and provide patients and families or caregivers with education and information pertinent to the diagnosis and plan of care.
  • Assist the Registered Nurse with skilled-needs procedures and duties, including preparing equipment and materials for treatments and helping patients learn appropriate self-care techniques.
  • Coordinate home health services and report identified needs for home health aides, occupational therapy, physical therapy, medical social work, and speech therapy to the Case Management Registered Nurse or other disciplines.
  • Use equipment and supplies effectively and efficiently.
  • Retrieve communications about assigned patients and use the information to establish care priorities.
  • Give timely and pertinent reports to the Case Manager.
  • Record concise information about patients' needs, problems, capabilities, limitations, and responses to nursing interventions.
  • Accurately record incidents affecting patients.
  • Report pertinent information promptly to the Case Management Registered Nurse and provider.
  • Attend weekly case conferences to coordinate and communicate patient-care and treatment information.
  • Maintain patient privacy in medical records and while providing care.
  • Maintain professional conduct and enhance clinical knowledge and skills.
  • Maintain productivity standards directed by the Case Management Registered Nurse.
  • Complete and review Start of Care assessments, documentation, and plans of care with the Case Manager as needed.
  • Participate in on-call duties as required by the weekend rotation.
  • Perform other duties assigned by the Case Management Registered Nurse.
