Requirements
  • An Associate degree in Human Services or a related field.
  • One year of experience working with community resources.
  • One year of experience generating care plans for clients or patients.
  • Community Health Worker certification.
  • Fluency in Spanish or Haitian Creole.
  • Ability to travel 25–30% of the work schedule.
  • Familiarity with Microsoft Office Suite, especially Outlook, Teams, and Excel.
  • Experience with electronic health record systems such as EPIC.
  • Ability to provide services using a strength-based, client-centered approach.
  • Demonstrated communication and interpersonal skills.
  • Ability to work collaboratively in a team environment or individually as needed.
Responsibilities
  • Provide initial and ongoing telephone and in-person support to clients and patients seeking assessment of social determinants of health, including basic needs, legal, medical, housing, transportation, and insurance needs, by offering and completing comprehensive needs assessments.
  • Collaborate with clients and patients and with behavioral health, primary care, dental, and other UCFS services to develop and implement patient-centered care plans that support client empowerment and improved overall health status.
  • Provide peer mentoring, coaching, skill building, and guidance relevant to consumer needs, including social skills, recovery supports, training, health awareness, and recreational activities.
  • Establish and facilitate client and patient communication plans, timelines, and follow-up.
  • Maintain familiarity with community resources to connect clients and patients with services, and collaborate with state and local agencies and other community-based supports.
  • Conduct community outreach to targeted groups and individuals to identify and address barriers to accessing and using health and social services.
  • Document care coordination activities and complete other record-keeping and administrative tasks required for reporting.
  • Organize and implement activities, including awareness events, speaking engagements, and dissemination of written materials.
