Requirements
  • Graduate of an accredited Master of Social Work program.
  • Hold a current Licensed Master Social Worker license in Texas.
  • Have field placement or internship experience in health services or with a health care provider.
  • Have effective oral and written communication skills.
  • Have working knowledge of the Diagnostic and Statistical Manual of Mental Disorders, Fifth Edition, and International Classification of Diseases, Tenth Revision manuals.
  • Demonstrate knowledge and skill in social work assessment and treatment of patients for mental health status and substance abuse screening.
  • Have therapeutic communication and negotiation skills when interacting with patients, families, physicians, and health care team colleagues.
  • Have strong analytical skills.
  • Have working knowledge or experience in utilization management, managed care, and payer issues.
  • Have exposure or experience in pre-acute and post-acute care and community resources.
  • Be able to work independently and develop collaborative relationships with physicians, families, patients, interdisciplinary teams, and community agencies.
  • Demonstrate knowledge and skills necessary to provide care appropriate to the patients served.
  • Demonstrate knowledge of growth and development principles as they relate to different life cycles.
Responsibilities
  • Systematically intervene to provide clinical social work and complex discharge planning to patients and families with complex psychosocial needs.
  • Assist with eligibility determination for social programs and funding sources and connect patients with community assistance from special funds and agencies.
  • Offer crisis intervention and mental health assessment to patients and families with psychosocial needs under the supervision of a licensed clinical social worker.
  • Coordinate and facilitate multidisciplinary discharge plans of care for high-risk patient populations.
  • Participate in an interdisciplinary team to provide services to at-risk individuals and address psychosocial issues across the continuum of care.
  • Assess patients' and families' psychosocial risk factors, including prior functioning, support systems, reactions to illness, and ability to cope.
  • Develop and carry out treatment plans using clinical social work diagnoses, assessments, and treatment interventions.
  • Intervene with patients and families regarding the emotional, social, and financial consequences of illness or disability and mobilize family and community resources.
  • Provide short-term individual, marital, and family therapy and crisis intervention under supervision when applicable.
  • Provide intervention involving child abuse or neglect, domestic violence, guardianship, institutional abuse, foster care, adoption, mental health placement, advance directives, adult or elderly abuse, child protection, and sexual assault.
  • Provide counseling and intervention related to treatment decisions and end-of-life issues.
  • Advocate for patient and family empowerment and independence in making health care decisions and accessing needed services.
  • Participate in complex patient discharge planning to support timely discharge and linkage with post-discharge care providers.
  • Address complex family dynamics that directly affect patient care and discharge.
  • Communicate discharge planning status to clinical care team members for referred patients.
  • Provide consultation to case managers when significant or intensive community resources are needed for treatment outcomes.
  • Receive referrals for complex patient problem resolution from case managers or clinical care team members.
  • Collaborate with clinical and case management teams on transition planning and referrals to post-acute care providers.
  • Keep clinical and case management team members updated on post-acute provider acceptance and discharge clearance.
  • Validate discharge criteria and notify clinical and case management teams of newly identified or changed resources.
  • Educate patients, families, and physicians about post-acute options and address issues of choice.
  • Ensure safe care for patients, staff, and visitors and follow Memorial Hermann policies, procedures, and standards.
  • Complete mandatory and continuing education and skills competency requirements.
  • Support department goals and serve as a preceptor, mentor, and resource to less experienced staff.
  • Provide compassionate, personalized, safe, and efficient experiences to patients and colleagues.
  • Perform other duties as assigned.

Memorial Hermann clinicians use Epic's AI tool to cut patient follow-up messages by 32%

Memorial Hermann Health System clinicians are using Epic's AI tool, Art, to help patients better understand their test results. The AI-powered assistant generates initial draft patient notes in clear, patient-friendly language as clinicians review lab and imaging results. Since implementation, patients have sent 32% fewer follow-up messages about test results when clinicians used Art's draft. The tool has helped Memorial Hermann's care teams write over 55,000 messages to patients, with clinicians saving an average of nine seconds per result comment. Memorial Hermann piloted the feature in December 2025 in their innovation clinic before expanding system-wide. Dr Ryan Walsh, chief medical information officer, emphasised that the technology is designed to support, not replace, clinician judgment and patient relationships.
