Requirements
  • Active LPN or Licensed Vocational Nurse (LVN) license.
  • Bilingual proficiency in Spanish and English.
  • One to two years of clinical experience in ambulatory care, population health, care coordination, or case management.
  • Strong clinical assessment, communication, and documentation skills.
  • Ability to work in an electronic health record (EHR) and chronic care management (CCM) platform simultaneously.
  • Ability to manage a structured, metric-driven workflow reliably.
  • Intermediate proficiency with Microsoft Office Suite, including Outlook and Teams.
  • Ability to work effectively in a primarily remote environment with home internet supporting at least 25 Mbps download and 10 Mbps upload speeds.
  • Ability to work from a dedicated space without ongoing interruptions to protect member protected health information (PHI) and HIPAA information.
Responsibilities
  • Perform monthly chronic care management (CCM) touchpoints for assigned patient panels.
  • Build ongoing relationships with patients and caregivers through regular outreach.
  • Review patient chart data, including diagnoses, kidney disease history, medications, labs, imaging, and treatment cycles, before calls.
  • Explain CCM participation and reinforce program benefits.
  • Conduct structured symptom assessments covering pain, fatigue, neuropathy, shortness of breath, nausea, and other symptoms.
  • Evaluate changes in patient condition using licensed clinical judgment.
  • Assess medication adherence issues, side effects, and confusion about medication regimens.
  • Identify clinical red flags requiring escalation.
  • Escalate urgent issues according to defined triage pathways.
  • Collaborate with the provider team regarding symptom changes, new concerns, and medication-related issues.
  • Route electronic health record messages with clinically relevant documentation.
  • Coordinate care by scheduling follow-up appointments, arranging labs, ensuring nephrology appointments are kept, and reducing missed care.
  • Reinforce provider-developed chronic kidney disease (CKD) and chronic disease care plans.
  • Provide clinical explanations of symptom-management instructions within LPN scope.
  • Support adherence to medications, lifestyle changes, and follow-up instructions.
  • Identify social, behavioral, or access barriers and determine appropriate interventions or referrals.
  • Document all patient interactions in the EHR and CCM patient management platform.
  • Track and log qualifying CCM time accurately for billing compliance.
  • Ensure documentation meets regulatory, internal quality-assurance, and partner expectations.
  • Maintain call quality and adherence to scripting and workflow pathways.
  • Maintain or exceed monthly engagement, billable conversion, and quality-assurance/documentation accuracy targets.
  • Participate in peer reviews, quality-assurance audits, coaching sessions, and continuing education.
  • Identify recurring patient needs and communicate trends to the Program Manager.
  • Perform other duties consistent with the role, as assigned.

  • Familiarity with CCM regulatory requirements.
  • Strong patient education and motivational interviewing skills.
