Requirements
  • A Nurse Practitioner must graduate from an accredited health professions school and successfully complete ANCC or AANP certification, or the candidate must be a board-certified Physician Assistant.
  • Nurse Practitioners with only PMHNP or NNP certifications are not eligible for this position.
  • Active licensure in applicable state or states is required.
  • Active DEA licensure and state-specific controlled substance registration are required, as applicable.
  • Basic Life Support certification is required.
  • Clinical experience caring for aging adults and adults with complex care needs is required.
  • Average to advanced computer and software skills are required.
  • The ability and means to travel in a timely manner to locations that may have limited access to public transportation are required.
  • Proof of liability and property damage insurance on the vehicle used is required.
  • Employees driving their own vehicle must have been licensed for at least five years and meet the stated driving-record requirements.
  • Frontline workers must be fully vaccinated and provide records for validation.
Responsibilities
  • Provide compassionate, individualized, and holistic patient care, including comprehensive care, urgent care, and chronic condition management, aligned with a value-based care delivery model.
  • Collaborate with the multidisciplinary team to deliver transitions-of-care support, social determinants of health interventions, behavioral health evaluation and management, and complex medication management as clinically appropriate.
  • Deliver clinical care both in person and virtually in private homes, congregate living facilities, ambulatory clinics, and skilled nursing facilities.
  • Integrate clinical practices, data dashboards, and operational processes supporting company performance goals into daily clinical workflows.
  • Promote and participate in patient engagement and patient experience initiatives.
  • Maintain comprehensive medical records, including timely and accurate clinical documentation and coding, medication reconciliation, and records of patient-care activities and communications.
  • Participate in care team huddles, operational huddles, organizational meetings, and clinical partnership meetings, including preparation and follow-up activities.
  • Engage collaboratively with internal care-team members and external providers and clinical partners to support care coordination and goals of care.
  • Follow medical policies, protocols, criteria, and clinical guidelines to maintain effective and efficient care delivery.
  • Follow compliance policies, procedures, standards of conduct, applicable laws, and regulations.
  • Initiate, develop, and sustain relationships with patients, caregivers, congregate living facility staff, and clinical partners as a growth and engagement ambassador.
  • Participate in the after-hours on-call schedule and provide clinical-care cross-coverage as requested.
  • Perform other market-specific duties as assigned.
Desired Qualifications
  • Advanced training in geriatrics or palliative medicine is preferred.
  • Clinical experience caring for patients in the home setting, including private homes or congregate living facilities, is preferred.
  • Previous experience with managed care or value-based healthcare delivery is preferred.
